 As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, you will work to help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
Primary Responsibilities:

<li>Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals</li>
<li>Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHC policy</li>
<li>Completes all patient evaluations and develops the PT plan of care within state specific guidelines</li>
<li>Reports outcomes of evaluation, goals, and anticipated projected frequency of care &nbsp;</li>

Required Qualifications:

Job DescriptionWe are currently seeking a licensed Physical Therapist (PT) and/or Physical Therapist Assistant (PTA) to provide in-home services for our growing home health agency. This role is ideal for experienced clinicians seeking flexible scheduling and consistent referrals. The contracted clinician will be responsible for performing new client evaluations and/or routine skilled therapy visits in a home health setting. Services are provided in accordance with physician orders, agency policies, and applicable state and federal regulations. Compensation 
<ul>
<li>Initial Evaluations: $95 per visit </li>
<li>Routine Therapy Visits: $85 per visit </li>
<li>Compensation is paid per completed visit (1099 Independent Contractor) </li>
</ul>
Responsibilities 
<ul>
<li>Perform comprehensive initial physical therapy evaluations (PT only) </li>
<li>Provide routine skilled therapy visits in the patients home (PT or PTA, per scope) </li>
<li>Develop, implement, and update individualized plans of care </li>
<li>Accurately document all evaluations, treatments, and patient progress </li>
<li>Communicate effectively with physicians, agency staff, patients, and caregivers </li>
<li>Ensure compliance with all South Carolina licensure requirements and agency policies </li>
<li>Educate patients and caregivers on treatment plans, safety, and home exercises </li>
</ul>
Qualifications 
<ul>
<li>Current South Carolina PT or PTA license in good standing </li>
<li>Minimum 1 year of clinical experience (home health preferred) </li>
<li>Knowledge of home health regulations and standards of care </li>
<li>Strong clinical judgment, communication, and documentation skills </li>
<li>Reliable transportation and valid drivers license </li>
<li>Professional liability insurance (or willingness to obtain) </li>
<li>Ability to work independently and manage a flexible caseload </li>

EDUCATION AND TRAINING
