We're seeking an experienced Inpatient Coder to join our remote Health Information Management team. This advanced coding role functions with a high degree of independence and requires strong analytical skills, coding accuracy, and clinical understanding across a wide variety of specialties.
You'll be responsible for accurately assigning ICD-10-CM/PCS diagnosis and procedure codes and MS-DRGs for inpatient hospital services across complex medical and surgical cases. This includes specialties such as Neurology, Oncology, Urology, Transplant, OB/Newborn, Orthopedics, Cardiology, and Critical Care — including trauma and acutely ill patients.

Job Summary and Responsibilities
Provides thorough concurrent, prospective, and retrospective review of ambulatory medical

guidelines, HCC standards, Risk Adjustment Factor (RAF) scoring, and physician query briefs.
Will participate in Provider education on the importance of diagnosis specificity and
documentation guidelines. The Risk Adjustment Coder works to maintain a thorough knowledge
of our current automated eClinicalsWork (eCW) enterprise billing system, through which the
coding and documentation review are functionalized to provide support to HCCH providers and
staffs as necessary. Provides subject matter expertise to others including staff in the Billing
department as necessary. This position requires professional maturity, responsibility, integrity,
and subject matter expertise to complete the work timely; communicate setbacks to deliverables.

Responsibilities include:
-Review and accurately code medical records and encounters for diagnoses and
procedures related to Risk Adjustment and HCC coding guidelines
-Validate and ensure the completeness, accuracy, and integrity of coded data.
-Concurrently, prospectively, and retrospectively review medical records to identify
unclear, ambiguous, or inconsistent documentation ensuring full capture of severity,
accuracy, and quality.
-Query providers when documentation in the record is inadequate, ambiguous, or
otherwise unclear for medical coding purposes.
-Utilizes approved resources to determine the appropriate ICD-10-CM, CPT, and/or
HCPCS and ensures documentation in the medical record follows official coding
guidelines, internal guidelines, and AHIMA physician query brief standards.
-Comply with the Standards of Ethical Coding as set forth by the American Health
Information Management Association and adhere to official coding guidelines.
-Comply with HIPAA laws and regulations.
-Maintain coding quality and productivity standards set forth by HCCH.
-Maintain competency in evolving areas of coding, guidelines, and risk adjustment
reimbursement reporting requirements.
-Assist in internal and external coding audits to ensure the quality and compliance of
coding practices.

Key Responsibilities
<ul>
<li>Review and accurately code cases to maximize reimbursement in a timely manner.</li>
<li>Review and accurately code E/M visits and office procedures.</li>
<li>Able to work independently and research coding scenarios.</li>
<li>Coder is responsible for meeting our daily production goal and our quality goal of averaging 95% accuracy rate on a consistent basis.</li>
<li>Attend conference calls as necessary to provide information and/or feedback.</li>
<li>Communicate with leadership on coding or documentation issues/trends.</li>
<li>Stay current on all coding guidelines (to include orthopedic-specific guidelines) and maintain credentials as necessary.</li>
<li>Participate in coding department and education meetings.</li>
<li>Flexible to expand coding skill set into other specialties altogether.</li>
<li>Maintain confidentiality and protect sensitive information.</li>
<li>Other duties as assigned by leadership.</li>
</ul>
Qualifications
<ul>
<li>High School diploma required. Associates or BS degree preferred.</li>
<li>Successful completion of at least one AHIMA or AAPC certified program with the achievement of the correlating professional credential (CCS, CPC, etc.); active and in good standing. A CPC or CCS-P certification is required. CPC-A is not accepted.</li>
<li>Minimum of 3 years physician coding experience (recent hands-on production) in professional general surgery coding &amp; procedural coding &nbsp;with expertise in Neurosurgery/Spine.</li>

Salary
This position offers a salary range of $27 to $34 per hour, commensurate with experience.
