The Case Manager provides direct and indirect patient care and case management services to active-duty patients receiving inpatient behavioral health treatment. *Pay Range: $22/hr. to $25/hr.
Essential Functions:

Education:

Licenses/Certification:

Job Functions:

<li>Provides direct psychosocial intervention.</li>
<li>Performs resident assessments at admission, upon condition change and/or annually.</li>
<li>Creates, reviews, and updates care plan and progress notes.</li>
<li>Provides direct psychosocial intervention.</li>
<li>Coordinates resident visits with outside services, dental, optical, etc.</li>
<li>Attends and documents resident council meetings.</li>
<li>Assists resident’s families in coping with skilled nursing placement, physical illness and disabilities of the resident, and the grieving process.</li>
<li>Works with the patient, family and other team members to plan discharge.</li>
<li>Conducts in-service programs to educate staff regarding psychosocial issues and patient rights.</li>
<li>Supervises and guides Social Services Assistants.</li>

About the Role:
We’re looking for a dedicated Social Services Director to lead our facility’s Social Services Department. In this role, you’ll oversee department operations in compliance with federal, state, and local regulations and ensure that each resident’s social and emotional needs are met on an individual basis. 
You’ll be instrumental in supporting residents and families through transitions, care planning, and community resources, while contributing to the overall success and occupancy of our community.
Key Responsibilities:

<li>Lead and manage the Social Services Department, ensuring compliance with all regulations</li>
<li>Assess residents’ social and emotional needs and develop individualized care plans</li>
<li>Participate in discharge planning and care coordination</li>
