Within this role, this person will provide patient care by removing the barriers to medication access. The Position:
The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting.
 
Responsibilities include:

<li>Provide the direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.</li>
<li>Evaluate and education patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.</li>
<li>Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.</li>
<li>Develop in conjunction with other team members the plan of care and evaluate response to therapy with each encounter.</li>
<li>Develop and maintain caseload scheduling via company approved platform per company policy. Communicate any scheduling changes on an as needed basis to appropriate team members.</li>
<li>Coordinate care via approved company platform</li>
<li>Takes part in the nurse on-call rotation to ensure continuous nursing support services to our customers.</li>
<li>Document all communications at time of service as appropriate per Soleo policy.</li>

<li>Work in conjunction with appropriate departments to ensure all required patient documents are completed and returned to the branch office within acceptable timeframe.</li>

Licensed Practical Nurses provide care for the residents in Assisted Living and act as a charge nurse for the Resident Aides. Licensed Practical Nurse does everything within their scope of practice per the OBN under the supervision of an Assisted Living Manager. 
What will I do as an LPN: Licensed Practical Nurse with Genacross?
<ul>
<li>Provides primary direction to the Resident Aides on assigned unit/floor/wing to ensure effective care is provided to residents.</li>
<li>Prepare and administer medications and treatments for residents.</li>
<li>Performs assessments i.e. vitals, intakes, outputs, etc.</li>
<li>Performs direct care functions (assists with ADLs &amp; ensures residents, spiritual, emotional and physical needs).</li>
<li>Processes physician's orders, labs, x-rays, etc. and seek out clarification when necessary.</li>
<li>Communicates appropriately with physicians, pharmacists, family members, etc. to discuss ongoing care of residents.</li>
<li>Process new resident admissions and communicate admission information to department heads, as appropriate.</li>
<li>Completes paperwork, medical forms, charting and all required documentation as a part of the resident care process and transition of care process.</li>
<li>Complete change of shift duties.</li>
<li>Assists in maintaining infection control program, policies and procedures.</li>
<li>Reports allegations of abuse/neglect and reports, as appropriate, to proper organization leadership.</li>
<li>Reports hazardous incidents, including but not limited to injuries, equipment, etc., to the appropriate Manager/Supervisor.</li>
<li>Implements and follows disaster plans in the event of an emergency.</li>
<li>Participates in committees as assigned.</li>
<li>Performs other job duties as assigned. </li>
</ul>
 
LPN: Licensed Practical Nurse Requirements:
<ul>
<li>Current State of Ohio RN/LPN License.</li>
<li>One to two years of experience in a long-term care setting is preferred.</li>
<li>Familiarity with basic computer skills. Ability to learn new software applications as required by the position.</li>
<li>Requires excellent oral and written communication skills, including the ability to interact with senior executives, managers, supervisors and direct care staff, as well as residents, families and physicians.</li>
<li>Understands directions; communicates and responds to inquiries promptly.</li>

 Responsibilities:

Skills &amp; Qualifications:

<li>Active Kentucy OR Multistate Pharmacist license in good standing.</li>

<li>Experience with Medication Therapy Management (MTM) preferred.</li>
<li>Strong attention to detail and communication skills.</li>
