Job Summary:
Assist the Social Services Manager and Social Service Coordinator in planning, organizing, and implementing the overall operations of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, established policies and procedures of The Sharon at SouthPark, and as directed by the Social Services Manager, and/or the Administrator, Manager or Director or Nursing to assure that the medically related physical, mental, emotional and social well-being of the resident is met/maintained on an individual basis.
Essential Functions:

Education and Experience:

<li>A baccalaureate degree in social work or a related human services field is strongly preferred.</li>
<li>The candidate must have a minimum of 6 months experience in a human services setting working directly with patients or clients. </li>
<li>1 -2 years' experience of the candidate working directly with the senior adult in a health care or assisted living setting strongly preferred.</li>
<li>Ability to work with and understand the needs of residents requiring minimal to total care.</li>

The CCS Individual Skill Development program uses goal-oriented interventions to support youth who are struggling in various aspects of their life, including social skills, emotional regulation, anger management, independent living skills, and behavior management.

PRIMARY FUNCTION/PURPOSE

The Social Services Supervisor is responsible for overseeing a team of Case Managers to ensure the support and resources necessary to promote self-sufficiency and independent living skills for families’ w/children residing in temporary shelter are being provided in accordance with CRF, DHS and OTDA standards.
PRIMARY RESPONSIBILITIES

<ul>
<li>Ensure document folder in CARES (VDP, Client rules and regulations, Crib acknowledgment, ACS Video, etc.) is completed and documented immediately upon client arrival. </li>
<li>Provide field orientation and on the job training to Case Managers. </li>
<li>Create and execute plans for meeting contracted ILP completion and placement goals. </li>
<li>Ensure all Case Managers complete mandated CRF, DHS, and OTDA tasks in accordance with prescribed timeframes and mandates. </li>
<li>Ensure Case Managers are providing consistent services in accordance CRF, DHS and OTDA policies and procedures. </li>
<li>Monitor Case Managers work activities via CARES reviews, physical chart reviews, and spreadsheets. </li>
<li>Conduct individual and group supervisions with Case Managers. Provide staff with documented feedback of performance, including areas of strength and areas in need of development. </li>
<li>Conduct performance evaluations of Case Managers as per CRF, DHS, and OTDA policies and professional standards. </li>
<li>Collaborate with Social Services team to develop service plans to address clients’ needs related to housing, substance abuse treatment, mental health and/or employment, etc. </li>
<li>Meet with Senior Management (PD and APD) for individual and group supervision. </li>
<li>Assist families with identifying appropriate housing resources and assist application process as needed. </li>
<li>Monitor demographic report to better guide Case Managers and ensure program compliance with CRF, DHS, and OTDA expectations. </li>
<li>Attend and moderate Case Conferences with Case Managers and challenging, noncompliant and/or housing ready clients. </li>
<li>Establish linkage with community service providers to obtain housing, health, and social services for clients. </li>
<li>Assist in facilitation of intake, discharge, transfers, and placement into permanency as needed. </li>
<li>Complete ILP’s and other Case Management related tasks as needed. </li>
<li>Generate and distribute reports (i.e., exit projection, quarterly projection, case summaries). </li>
<li>Support in the acquisition of housing vouchers and placements. </li>
<li>Deliver crisis management and intervention services to clients. </li>
<li>Support efforts to place families into permanent housing. </li>
</ul>

MINIMUM QUALIFICATIONS

<ul>
<li>Bachelor’s degree in Social Work required.</li>
<li>At least two (2) years of experience in a related field. Experience with multi-problem families desirable.</li>
<li>Bilingual in Spanish. </li>
<li>Demonstrated knowledge of and proficiency in CARES. </li>
<li>Demonstrated experience working in shelter setting and/or with high-risk families, including families with documented histories mental health, substance abuse, Domestic Violence, child abuse/neglect. </li>
<li>Ability to work effectively with a wide range of community-based providers.</li>
<li>Working knowledge of Case Management theories and approaches.</li>
<li>Knowledge of community health services, social service support agencies and networks.</li>
