Position Summary: 
Will collaborate with all staff for the care of behavioral health patients both inpatient and outpatient needs of the patient, the facility, and clinic. 
Essential Functions (Responsibilities/Accountabilities):

<li>Will collaborate with multidisciplinary team to help meet the needs of individual patient, especially when coordinating social disparity needs of the patient. </li>
<li>Provides psychosocial assessments for inpatient and outpatient referrals, i.e. assess patient’s social, emotional, environmental, financial and support needs.</li>
<li>Demonstrate some knowledge of medical terminology.</li>
<li>Implements interventions during crisis situations for the emergency department and critical care unit. </li>
<li>Collaborates with physicians, nurse practitioners and other providers regarding a patient’s care.</li>
<li>Provide patients with recommendations and provide referrals to other departments or community agencies as needed.</li>
<li>Facilitate the hospital’s bereavement, cancer and caregiver support groups as needed.</li>
<li>Complete assessments and documentation in a timely manner.</li>
<li>Assist with discharge planning as needed for outpatient referrals.</li>
<li>Advocate for patient’s human rights to be acknowledged in all decision-making.</li>
<li>Help patients/families adjust to hospital admission; possible role changes; exploring emotional/social responses to illness and treatment.</li>
<li>Effectively communicates with physicians, admitting office, medical records, staffing coordinator, and nursing staff to initiate patient care and enhance positive outcomes.</li>
<li>Assists with hospital and community education with clinical issues and participates in community events.</li>
<li>Works conjunction with a multidisciplinary team assess patient’s condition, formulates and implements individualized treatment plan and serves as a clinician and case manager.</li>
<li>Maintains knowledge of current empirically supported treatments and provide coverage for personal growth and psycho education groups when needed.</li>

Job Summary:
Assist the Social Services Manager and Social Service Coordinator in planning, organizing, and implementing the overall operations of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, established policies and procedures of The Sharon at SouthPark, and as directed by the Social Services Manager, and/or the Administrator, Manager or Director or Nursing to assure that the medically related physical, mental, emotional and social well-being of the resident is met/maintained on an individual basis.
Essential Functions:

<li>Assist with and support the initial resident admission process, as directed, to facilitate a positive move in experience for the resident to healthcare or assisted living.</li>
<li>Engages with the healthcare and assisted living residents daily to help them adjust positively to living in the facility.</li>
<li>Responsible for the planning, development, and implementation of assigned Social Services documentation, such as admission / quarterly assessments, progress notes, care plans, behavior progress notes, BIMS, in the EHR (electronic health record) in accordance with current federal, state, and local standards, guidelines, and regulations and department policies of The Sharon.</li>
<li>Ensure that all documented social services progress notes in the EHR (electronic health record) are informative and descriptive of the services provided and of the resident's response to the service.</li>

Education and Experience:
<ul>
<li>At a minimum a high school diploma is required. </li>
<li>A baccalaureate degree in social work or a related human services field is strongly preferred.</li>
<li>The candidate must have a minimum of 6 months experience in a human services setting working directly with patients or clients. </li>
<li>1 -2 years' experience of the candidate working directly with the senior adult in a health care or assisted living setting strongly preferred.</li>
<li>Ability to work with and understand the needs of residents requiring minimal to total care.</li>
<li>Ability to communicate effectively; verbally and in writing.</li>
<li>Ability to listen and respond to the needs of the residents.</li>
<li>Strong customer service skills.</li>
<li>Ability to function effectively with a team environment.</li>
<li>Willingness to work in an environment that changes daily based on the needs of the residents. </li>
<li>Proficient computer skills in Microsoft Office (Word, Excel, PowerPoint, Outlook) plus the willingness to learn other computer programs as required.</li>
