HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:

<li>Location: Fayetteville WV 25840 – 100% On-site</li>
<li>
<ul>
<li>Counties Covered: Nicholas and Fayette</li>
</ul>
</li>
<li>Payrate: $57.30/hr + mileage</li>
<li>Required Availability: Full-Time | Monday–Friday, 8:00 AM–5:00 PM</li>
<li>

<li>On-Call: One Saturday per month</li>
</ul>

HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:
The Home Health Physical Therapist role provides in-home rehabilitation services to help patients improve mobility, function, and overall independence. The therapist evaluates patient needs, develops individualized treatment plans, and collaborates with the care team to deliver high-quality, compliant care across the assigned service area.

<li>This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs</li>
<li>Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance</li>
<li>A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates</li>
</ul>
HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:
<ul>
<li>Perform comprehensive physical therapy evaluations and treatments based on physician referrals</li>
<li>Review patient medical histories, diagnoses, and precautions to develop individualized care plans</li>
<li>Establish measurable goals and treatment plans to support functional independence</li>
<li>Reassess patient progress regularly and adjust treatment plans as needed</li>
<li>Communicate patient progress and needs with physicians, care teams, patients, and families</li>
<li>Recommend and assist with adaptive equipment to enhance rehabilitation outcomes</li>
<li>Maintain accurate and timely clinical, treatment, and billing documentation in compliance with all regulations</li>
<li>Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice guidelines</li>
<li>Participate in staff development, meetings, and continuing education to maintain clinical excellence</li>
</ul>
HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:
<ul>
<li>Education: High School Diploma or GED (verifiable)</li>
<li>Attendance: Commitment to mandatory attendance during the first 90 days.</li>
<li>Active West Virginia Physical Therapist license</li>
<li>Compact licenses accepted</li>
<li>Home health experience required</li>

Responsibilities
•&nbsp;&nbsp; Complete evaluations, assessments, and treatments in accordance with policy and the individualized physical therapy care plan
•&nbsp;&nbsp; Complete all documentation and records in a timely and accurate manner
•&nbsp;&nbsp; Follow established policies and procedures as related to the scope of practice
•&nbsp;&nbsp; Recognize and respond to priorities in patient care showing initiative and sound judgment
•&nbsp;&nbsp; Complete all required orientations, in-services, and trainings successfully and in a timely manner
•&nbsp;&nbsp; Perform other related duties as necessary and as directed by supervisor
 
LICENSURE, CERTIFICATION AND EDUCATIONAL REQUIREMENTS
•&nbsp;&nbsp; Valid professional license in the state of practice
•&nbsp;&nbsp; Graduate of CAPTE accredited school.&nbsp; If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credential’s evaluation organization approved by the APTA
