Job Summary
Under the general direction of the Chief Medical Officer, the Medical Director of Crisis Continuum Services provides clinical and administrative leadership for OCHN’s crisis system, including Crisis Assessment &amp; Triage, Crisis Stabilization Unit (CSU), Crisis Residential Unit (CRU), Sober Support Unit, Mobile Crisis, and Crisis Line/Hotline. This physician will lead the redesign and ongoing operation of an integrated, evidence-based crisis continuum, set clinical standards, and serve as final medical authority for crisis-related determinations. Recommended allocation: 50% clinical / 50% administrative (alternative track available: 75% clinical / 25% administrative depending on program volume and leadership coverage). 
Essential Functions
Clinical Care and Oversight
<ul>
<li>Performs all duties and responsibilities of a psychiatrist; provides high-quality, cost-effective care consistent with licensing, accreditation, and payer standards. </li>
<li>Conducts and/or supervises psychiatric evaluations, medication management, suicide and violence risk assessments, and substance intoxication/overdose/withdrawal assessments (including the use of structured assessment tools and symptom screening questionnaires); within crisis workflows determines level of care (e.g., ED transfer, inpatient, CSU, CRU, partial hospital, outpatient). </li>
<li>Provides final clinical disposition decisions and physician certifications as required (e.g., involuntary petitions/clinical certifications under the Michigan Mental Health Code). </li>
<li>Ensures timely documentation in the EHR and adherence to HIPAA and 42 CFR Part 2.</li>
</ul>
Leadership and Program Direction
<ul>
<li>Serves as the lead medical authority across the Crisis Continuum; designs protocols, care pathways, and escalation algorithms consistent with SAMHSA Crisis Guidelines and MDHHS expectations. </li>
</ul>
<ul>
<li>Leads the build-out of Crisis Assessment &amp; Triage services, integrating psychiatrists and Advanced Practice Providers (APPs) with nurses, bachelor’s-level clinicians, counselors, social workers, and psychologists, leading this multi-disciplinary team to deliver rapid, reliable, evidence-based care and disposition determinations. </li>
<li>Partners with internal teams, hospitals/EDs, law enforcement, EMS, and community providers to ensure warm handoffs and least-restrictive, recovery-oriented care transitions. </li>
</ul>
Supervision &amp; Collaboration
<ul>
<li>Serves as collaborating physician of record for APPs; provides regular chart audits, scope-of-practice oversight, and timely case consultation.</li>
<li>Leads daily/shift huddles and interdisciplinary rounds (nursing, crisis clinicians, peers, behavioral health technicians (BHTs)/milieu staff, discharge planners) to resolve barriers, update risk, and finalize dispositions. </li>
<li>Provides clinical supervision to medical students, residents, and fellows.</li>
</ul>
Quality, Compliance, and Performance
<ul>
<li>Designs and monitors quality measures and key performance indicators (KPIs) (e.g., time to triage, time to disposition, 23-hour utilization, return-in-crisis rates, warm handoff completion).</li>
<li>Ensures compliance with all relevant OCHN policies/procedures, MDHHS/Medicaid requirements, accreditation standards, and the Michigan Mental Health Code; participates in Root Cause Analyses and special reviews. </li>
<li>Oversees utilization review for level-of-care decisions, second opinions, and re-evaluations for individuals awaiting placement. </li>
<li>Serves on OCHN Medical Directors’ Advisory Group (MDAG) and other committees as directed by OCHN Chief Medical Officer.</li>
</ul>
Education &amp; Workforce Development
<ul>
<li>Develops and delivers training (risk tools, de-escalation, medication algorithms, triage workflows); helps develop curricula for medical student, resident, and fellow rotations (in addition to clinical supervision). </li>
<li>Promotes a culture of person-centered, trauma-informed, and recovery-oriented practice across teams. </li>
</ul>
Administrative Responsibilities
<ul>
<li>Co-leads strategic planning, development of staffing models (psychiatrists/APPs), scheduling, and coverage plans for 24/7 operations; contributes to budgeting, grants, and regulatory reporting. </li>
<li>Coordinates with OCHN Core Provider Agencies (CPAs), Medicaid Health Plans, and other community providers to support integration and care continuity. </li>
</ul>
Job Requirements and Qualifications
Education: 
<ul>
<li>MD or DO from an accredited medical school.</li>
</ul>
Training Requirements (licenses, programs, or certificates): 
<ul>
<li>Board Certified (or eligible) in Psychiatry; Michigan medical license; Michigan Controlled Substance License; DEA registration; BLS.</li>
<li>Preferred additional fellowship training and board certification (e.g., Addiction Psychiatry, Child and Adolescent Psychiatry, Consultation-Liaison Psychiatry, Forensic Psychiatry, or Geriatric Psychiatry).</li>
</ul>
Experience Requirements:
<ul>
<li>Minimum 5 (five) years post-residency clinical experience, including crisis/emergency psychiatry.</li>
</ul>
Preferred Requirements:

<li>Prior supervisory/leadership experience is preferred.</li>

The Emergency Department is a state-of-the-art, 22-bed Level 3 Trauma unit that opened to the public in 2014. The ED handles critically ill patients and other emergency medical issues, including OB/GYN, geriatric, and medical-surgical including bariatric, orthopedic, oncology, and stroke patients. The majority of ED nurses at Magee are trained to perform forensic exams for sexual assault as well. Interim Housing is a county-funded Department of Mental Health (DMH) program for unhoused individuals with severe mental health problems. The Position
The Resident Aide ensures that the interim housing site is a safe, healthy and therapeutic environment. Resident Aides are responsible for assisting clients with increasing independence with activities of daily living and assists clients in adhering to program policies and procedures. POSITION SUMMARY
A Forensic Nurse Examiner is specially trained and certified by the Maryland Board of Nursing to provide comprehensive care to adults, adolescents and children who have been impacted by sexual assault, intimate partner violence, child abuse, human trafficking and non-intentional trauma. The Examiner works independently under the general direction of the Forensic Nurse Examiner Manager.
The following job description covers the most significant duties performed, but does not exclude other work assignments not mentioned.
PRIMARY RESPONSIBILITIES
Principal Duties:
1. Performs forensic evidentiary examinations on victims and alleged perpetrators in connection with chronic or acute physical, sexual, or domestic assaults, and specifically:
a. Provides timely, non-judgmental, compassionate care to patients with a complaint of sexual assault utilizing a clinical process.
b. Obtains consent from the subject or custodial/proper authority prior to the examination relative to photography and evidence collection.
c. Performs evidentiary physical assessments using ALS when indicated based on evidence-based research and unit-based training.
d. Completes the Physical Evidence Kit required by law enforcement authorities.
e. Photographs patients and injuries as training indicates using proper technique.
f. Ensures compliance with Chain of Custody requirements. 2. Interfaces with law enforcement officials, crime labs, prosecutors, and other authorized stakeholders concerning forensic processes and outcomes to include providing expert testimony in judicial proceedings.
3. Provides testing, prophylactic treatment, prescription refills and related education to patients who potentially may have been exposed to HIV during their assault.
4. Participates in peer reviews, staff meetings and SART meetings.
5. Performs related work as assigned.

1. Greets customers in courteous, friendly, respectful and professional manner at all times, including maintaining eye contact when appropriate.

Forensic ACT Team Psychiatric Nurse

The Forensic Assertive Community Team (FACT) Psychiatric Nurse is a clinical staff member of the FACT
Community based multidisciplinary treatment team. The Nurse has the lead responsibility for assessing physical health needs; making appropriate referrals to community physicians; collaborating with medical providers and pharmacies; teaching wellness skills; and providing management and administration of medication under the direction of the team Nurse Practitioner. Additionally, the Nurse is responsible for providing a full range of services that are culturally relevant and recovery based. These services may occur in the home, community, or office. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, and providing direct assistance and securing necessary community resources. 
1. Develop a working knowledge of FACT as an evidenced based model and practice according to
the guidelines set forth by OMH. Participate in daily morning meetings with the purpose of reviewing client progress/needs, creating daily visit schedule and developing team relationships.
2. Conduct person-centered comprehensive health assessments, including personal and family medical and psychiatric histories, current medical needs, substance use history, and daily living skills with the purpose of linkages to and collaboration with providers in order to improve health outcomes. 
3. Develop and implement strength-based treatment plans that are responsive to client needs and facilitate achievement of goals on an as needed basis.
4. Oversee all aspects of medication management in compliance with agency procedures and 
protocols. This includes administration of injectable medications, managing Clozaril protocols, ordering of medication, overseeing processes for medication distribution, utilizing a medication step system towards client self-management, and managing all aspects of the medication room.
5. Provide support, treatment, case management and community advocacy and linkage services, per
the treatment plan. Complete required documentation of interventions as well as client’s responses and outcomes.
6. Readily respond to all crisis situations and urgent needs using sound clinical judgement in order to ensure the safety and well-being of the individual. Provide on-call crisis intervention services on a rotating schedule amongst team members. Always act to ensure client’s safety, readily assess needs, provide support, and intervene immediately as needed.
7. Mohawk’s programs and offices are multi-level with stairs only. Individual services in the community include visits to individuals’ apartments which can be on the second and third floors. Employees must be able to travel to all levels of the facility in a safe and reasonable time frame.
8. Provide transportation to individuals when necessary. Requirements
Skills/Qualifications for the Forensic ACT Team Psychiatric Nurse include:
1. Licensed Registered Professional Nurse by New York State.
2. Bachelor’s degree in nursing preferred.
3. Three years’ experience in a psychiatric setting, with at least one year’s experience in an outpatient setting is preferred.
4. Must have a strong commitment to the rights and ability of each person with a mental illness to make own decisions and choice, live in the community, work, and have access to continuous support and services to enhance wellness.
5. Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.
6. Must uphold individuals’ rights and personal preferences in treatment and treat everyone with respect and dignity. Continuously acting as a guest in individuals’ homes and family homes, is courteous, polite and an active listener.
7. Valid and insurable driver's licenses. Must use your own vehicle for business. Agency vehicles are available and need to be used to transport individuals.

The Staff RN - SANE (Sexual Assault Nurse Examiner) is responsible for completing sexual assault interviews and physical assessments. Communicates and collaborates with hospital personnel, advocacy and law enforcement representatives.

RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
\n\n
*Commits to the mission, vision, beliefs and consistently demonstrates our core values.

*Collaborates with the assigned Emergency Department (ED) Staff RN to provide care to the sexual assault patient.
\n\n
*Completes sexual assault interviews and physical assessments according to procedure and as warranted by patient condition.
\n\n
*Conveys respect to and assures privacy, confidentiality and security for victims of assault.
\n\n
*Performs physical examinations including pelvic exams with colposcopic pictures; explains all aspects of the examination and care provided.
\n\n
*Collects forensic evidence while maintaining integrity and security of the assault evidence kit and collected specimens/samples in accordance with the State of Nebraska protocol and law.
\n\n
*Consults with the Emergency Department physician regarding findings; seeks direction for treatment of additional injuries (e.g. sutures).
\n\n
*Collaborates with members of Sexual Assault Response team member i.e., Advocacy and Law Enforcement; accesses crisis counselor/advocate.
\n\n
*Performs therapeutic nursing interventions including:
\n\n
<ul>\n\t
<li>\n\t Providing patient/family education and emotional support;\n\t</li>
\n\t
<li>\n\t Referring to supportive community agencies; and,\n\t</li>
\n\t
<li>\n\t Involving the patient in the treatment plan and decision-making.\n\t</li>

*Meticulously documents the findings and care provided including:
\n\n
<ul>\n\t
<li>\n\t physical and psychosocial assessment;\n\t</li>
\n\t
<li>\n\t identified patient needs and associated nursing interventions;\n\t</li>
\n\t
<li>\n\t patients response to interventions; and,\n\t</li>
\n\t
<li>\n\t discharge status.\n\t</li>

QUALIFICATIONS:
\n\n
Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations. Minimum of one (1) year clinical nursing experience required.Basic Life Support (CPR) certification required. Completion of SANE didactic and practicum education required.
