<li>Administers resident care utilizing the nursing process in a proficient and professional manner consistent with current standards of practice.</li>

<li>Transcribes and notes orders per facility standards; communicates orders to other departments as appropriate; schedules follow-ups as directed.</li>
<li>Provides assessments of resident conditions as needed, documenting findings and updating care plans accordingly.</li>

Resident Safety and Clinical Quality

MDS Coordination
<ul>
<li>Coordinates completion of MDS with all disciplines in accordance with regulations.</li>
<li>Maintains the assessment schedule to comply with regulatory standards.</li>
<li>Performs and documents nursing assessments necessary to develop an accurate MDS.</li>
<li>Completes and reviews the nursing component of the MDS, ensuring accuracy and timeliness.</li>
<li>Provides education, coaching, and training to care plan team and support staff to assure accurate documentation and MDS coding.</li>
<li>Coordinates with therapy services to ensure appropriate documentation supports services provided.</li>
<li>Monitors resident conditions to determine if a change of condition MDS is required.</li>
</ul>
Care Planning and Oversight
<ul>
<li>Assists in the development and implementation of individualized resident care plans.</li>
<li>Incorporates input from residents, families, and staff to ensure comprehensive, person-centered plans.</li>
<li>Monitors effectiveness of care plans, makes modifications as needed, and ensures documentation reflects care provided.</li>
<li>Contributes to and supports interdisciplinary care planning meetings.</li>
</ul>
Regulatory and Compliance
<ul>
<li>Assists the DON in ensuring compliance with state and federal regulations, including Long Term Care Regulations</li>
<li>Adheres to facility policies, safety standards, infection control practices, and compliance program requirements.</li>
<li>Participates in audits, surveys, and quality improvement initiatives.</li>

Requirements:

<li>Registered Nurse (RN) licensure is required in state of practice.</li>
<li>LPN or LVN licensure is required in state of practice.</li>
<li>Minimum of 3 years' experience as an RN or LPN in assisted living, skilled nursing, or a related field is required. </li>
<li>Minimum of 3 years leadership experience in a healthcare setting, preferably in assisted living, skilled nursing, or a related field is preferred. </li>
