Department: BH Forensic ACT-Tech 
Forensic Assertive Community Treatment (FACT) is an evidenced based model that provides wrap around mental health services in the community to adults experiencing persistent and severe mental health issues, and criminal justice involvement. The FACT RN is a part of a multidisciplinary team and is primarily responsible for providing medication support and physical health appointment coordination support to clients. FACT RN is also responsible for managing the FACT medication room. Most of the services provided by the FACT RN take place in the community, MDC, and other nontraditional settings which includes clients’ homes and shelters.&nbsp;

Position Summary: Provide professional nursing care for assigned patients in a mental health/psychiatric environment. Design nursing care plans and assist in the development of overall care plans for psychiatric patients, ensuring administration of appropriate overall care in a manner conducive to recovery. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include pediatric, adolescent, adult and geriatric age groups.

Key Responsibilities
<ul>
<li>Provides direct psychiatric care to patients via onsite and telepsychiatry services for scheduled appointments, emergency, or urgent care visit.&nbsp;</li>

Glassport LTSR is a long-term structured residence that serves males with a forensic and mental health history. Our goal is to reintegrate these residents back into the community. We hope to accomplish this goal by working on activities of daily living, such as cooking, cleaning, and budgeting. We also link the residents to service providers who can also help them build relationships in the community. We strive to provide holistic care through group and individual therapy, medication management, psychiatric rehabilitation activities, and evidence-based practices. 
This is a full time position and the shifts are Monday through Friday 3 pm to 11 pm.
Responsibilities:
 Complete daily mental status exams on 6–12 residents. 
 Complete narcotic counts at beginning and end of shift. 
 Administer medication via medication administration record. 
 Identify early signs of psychiatric relapse, medication side effects, or medical issues. 
 Maintain detailed nursing notes in EMR/charting. 
 Document progress toward treatment plan goals. 
 Ensure compliance with state regulations, Joint Commission, and organizational policy. 
 Lead response to psychiatric or medical emergencies. 
 Make patient appointments and assist in transportation when needed. 
Clinical 
 Collaborate on Personal Recovery Plans which emphasize residents’ strengths and needs.

<li>Relevant experience working with Individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement, </li>
<li>A current driver’s license and California automobile insurance coverage for driving your personal vehicle to and from various community locations either independently or with other team members and/or Individuals, and if a company vehicle is available for use, one must also be 21 years of age, and </li>
<li>Ability to read, write and speak English. Bilingual in English/Spanish is preferred.</li>

There will be challenges:
<ul>
<li>You will see many types of disabilities and mental health challenges in the Individuals (aka the Program Participants) that we serve. You will also learn about the different agencies and resources we work with to help you provide support directly, or indirectly, to the Individuals. Because you care, the Individuals can improve their quality of life and reach their goals.</li>
</ul>
 
There will be chores: 
<ul>
<li>You will learn to take a customized, “person-centered” approach by leveraging your knowledge of the Individual’s abilities, medical condition, history, and their Individual Service Plan as you work to provide Individuals choices, instruction, and guidance in developing social and/or job-related skills. </li>
<li>You will train Individuals on task-level actions to perform their job duties/assignments effectively and in a timely manner and coach them to have positive interactions with others, e.g., teaching socialization and job etiquette skills, etc. as they work at a job or participate in work-type experiences in the community. You will also be required to keep accurate daily notes and provide input to personalize or modify their training needs and/or schedule as needed.</li>

<li>You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the Family. We like to promote from within our own house!</li>
<li>You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).</li>
<li>You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.</li>
</ul>
