Clinic Nurse 
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES: The clinic nurse works as part of the clinic provider team and assists the providers in the provision of primary and specialized health care. This position provides direct patient services and performs specifically designated independent procedures. This position monitors patient progress and coordinates clinic and community resources to assist patients /families in receiving high quality, evidence-based, cost-effective care and achieving patient-focused outcomes. The work includes the responsibility for significant interpersonal staff and public contact, appropriate and professional conduct, professional attire, independent judgment, discretion, and personal initiative as well as resolution of complex problems. Excellent communication skills and the ability to consistently maintain professional composure is an absolute must for this position.
Specific Duties and Responsibilities: 
A. Patient Relations: 
a. Prepares patients for examination including the identification of the chief complaint, the taking of vital signs, height, weight, etc., and ensures smooth patient flow throughout the clinic. 
b. Serves as system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf to ensure approval of the necessary supplies/services for the patient in a timely fashion. 
c. Ensures that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health. 
d. Performs patient teaching, patient self-management/self-efficacy coaching and planned care management. Ensures coordination for specialty services, 340B pharmacy program benefits, behavioral health services and other services as necessary. 
e. Serves as a liaison between patient, PCP, insurance companies, specialty, and behavioral health providers 
f. Assists patients in accessing their health care benefits, i.e. transportation, 340B medications and supplies, community resources, interpretation services, etc. g. Ensures patient confidentiality. h. Routinely provides patients with other clinic and community-based resource materials as appropriate. i. Ensures patient confidentiality. j. Provides patient education, per periodicity of life cycle, or as requested by patient. k. Reviews health summary including patient’s current problem list and past medical history to ensure that updates have occurred annually. l. Other duties as assigned. 
B. Staff Relations/Team Support: a. Assists all providers in the healthcare activities, performance of diagnostic and/or therapeutic procedures, and the administration of medications as necessary. b. Prepares exam rooms for patients throughout the day. d. Maintains working knowledge of 340B Pharmacy Program to assist and provide guidance to patients e. Performs other necessary duties as required by the Medical Director, Director of Quality, and Quality Assurance Reviewers to meet the goal of providing primary healthcare services. f. Assists with taking patient calls and triage per protocol, schedules referrals for appointments and procedures. g. Participates in the community health center’s quality improvements activities and performs duties in accordance with applicable standards. h. Helps to implement goals, objectives, policies, procedures, and systems such as Patient Centered Medical Home (PCMH), Meaningful Use (MU), and other programs or objectives for HCI. i. Supports and helps to achieve HCI’s clinical and business work plan in accordance with standards required to meet Patient Centered Medical Home recognition and other programs or objectives for HCI. j. Develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support quality care. 
C. Clinical Duties: a. Conducts patients screening for vision, blood pressure, vision screening and other simple routine procedures. b. Collects specimens and delivers them to the laboratory. c. In the absence of a laboratory technician, performs routine laboratory procedures per training competency. d. Perform individual emergency treatment while already providing or undertaking covered services. e. Collects patient and family medical and behavioral health history. f. Evaluates social and cultural needs, preferences, strengths, and limitations. g. Identifies whether a patient has specific communication requirements due to hearing, vision, or cognition issues. 
D. Administrative Duties: a. Maintains licensure required to perform duties including any CEUs required for licensure. b. Maintains all patient records to comply with required standards, c. Maintains HIPAA compliance. d. Complies with all HCI policies, to include but not limited to, the HCI Employee Handbook. 
E. Safety/Infection Control Activities: a. Observes all safety/infection control policies and procedures. b. Completes annual TB testing. c. Attend safety/infection control committee training as required. d. Ensures an appropriate environment for the administration of medical care by keeping exam rooms, stations, all equipment, and other work areas in a clean, safe, and orderly fashion. e. Work is performed in an interior medical/clinical environment. f. Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day. g. Work environment involves exposure to hazards or physical risks, which require following safety precautions. h. Will work with blood or blood-borne pathogens and will require OSHA training. 
F. Corporate Compliance a. Employees are encouraged to support and cooperate with the Healthy Connections, Inc. G. Reporting Relationship The Clinic Nurse reports to and is supervised by the Clinical Territory Lead and Director of Nursing.

Every employee is required to check and respond to company email daily to promote prompt and efficient communication and to become educated on the business of the organization. 
I. Evaluation The evaluation of work performance will be on-going and will be completed by the Clinical Territory Lead and Director of Nursing. It will include the specific duties and responsibilities of this position description plus employee ability to interact constructively with clinical and administrative staff and general working behavior. Formal evaluation will be conducted annually but may be initiated at other times by the Medical Director. 
J. Qualifications: a. Certification of practice as a Licensed Practical Nurse or Registered Nurse. b. Ability to work flexible schedule to meet the needs of the clinic. All employees are required to work shifts that meet the needs of the clinic. Shifts may be any day the clinic is open and the deemed necessary to best serve our patients. c. Must be qualified in Basic Life Support techniques. d. Must have a working knowledge of Microsoft office products, and familiarity with network systems. e. Ability to understand and present verbal instructions and to exchange verbal information is essential. f. Excellent organizational, written and customer service skills necessary. g. Ability to travel to all locations and to meetings outside of the service area. h. This 2 operatory practice is fully digital, has&nbsp;intraoral cameras, Fotona laser, and&nbsp;is open to&nbsp;additional equipment depending on physician preference and skillset.The practice accepts mostly PPO/FFS,&nbsp;offers Medical, dental, vision, 401K matching, and a&nbsp;CE stipend.&nbsp;Compensation for this role includes a daily rate and/or a % of net production along with bonuses.&nbsp;The position schedule would be ﻿8 am to 5 pm, Monday - Friday.&nbsp;This is a great opportunity to get into a busy practice and make it your own!﻿

Your Role:

<li>Conduct exams, screenings, and comprehensive dental treatments (restorative, endodontics, oral surgery) for military personnel.</li>
<li>Utilize mobile equipment to deliver quality care in non-traditional settings.</li>

Requirements:
Qualifications:

<li>Licensure &amp; Certification: Active, unrestricted state dental license and CPR/BLS certification.</li>
<li>Preferred Skills: Experience with mobile dental equipment, military experience a plus, and computer proficiency.</li>
<li>Availability: Participation is flexible and completely up to you—choose events that fit your schedule, with a focus on weekend availability to support military readiness</li>
</ul>
