The Care Manager plays a pivotal role in coordinating and managing comprehensive care plans for individuals requiring long-term support within the healthcare and social assistance sector. The Care Manager collaborates closely with multidisciplinary teams, including healthcare providers, social workers, and community resources, to facilitate access to Medicaid and other support services. Minimum Qualifications:

JOB SUMMARY:

Primary Job Responsibilities:
