This office services adult and geriatric patients in homes throughout Addison and Chittenden County.
Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required. 
As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Speech Language Pathologist, SLP Responsibilities:

Position Summary 
We are seeking a compassionate and dedicated Permanent Supportive Housing (PSH) Lead Case Manager (LCM) to support individuals and families experiencing homelessness in Washington, DC. In addition to providing case management to residents in PSH units across the city, this position supports the PSH Director, Clinical Manager, and the Case Manager Supervisor in problem solving, resource management, and reviewing/auditing case files for continuous quality assurance. The LCM promotes a cooperative team environment and fosters a focus on excellence and increased service quality. The LCM will also maintain a full caseload, providing intensive case management services to promote housing stability, recovery, and increased independence. Clients may have been diagnosed with a mental or physical health disability, lack support networks and access to mainstream resources and/or have experienced trauma such as domestic violence.
In this role, the Lead Case Manager is focused on two main objectives – to serve as a team leader and to provide case management to program participants.
Essential Functions Team Lead

<li>Assist the supervisor with training and on-boarding for new and existing employees, which includes accompanying team members on field visits and the proper use of HTH and HMIS.</li>

Hours Per Week: 40 hrs / week during training
Schedule: Monday - Friday 8am - 4pm during training
SUMMARY:
This role is a transitional, entry-level position designed to train the trainee on the necessary skills required to become a Certified Nursing Assistant (C.N.A). Candidates hired into this position are required to take and pass the C.N.A Training Class and state mandated test in order to earn their C.N.A Certification. Once classes have been successfully completed, employee may begin working as a nursing aide until certification is obtained.
RESPONSIBILITIES:
· Skillfully and safely, perform all activities required to successfully pass the C.N.A certification in order to transition into the C.N.A role.
· Must remain in good standing with training program. Failure to remain in good standing for any reason may result in termination at the discretion of the training program leaders.
· Read and follow resident’s care card. Care plan includes residents daily personal care needs, treatments and therapies. Engage nurse with any questions related to care plan.
· Gives personal care to residents: bathing, toileting, feeding, transferring, and ambulating.
· Promote residents involvement and independence to the extent the resident is able.
· Observe and report changes in physical condition, behavior, reactions, and appetite to nurse. Report any hazardous conditions that may jeopardize resident, visitor, or staff safety.
· Transport residents throughout the facility as needed.
· Attend in-services to fulfill the number of hours required by state.
· Perform other duties as assigned.
REQUIRED QUALIFICATIONS:
· C.N.A. Certification required to be obtained within 6 months of hire.
PREFERRED QUALIFICATIONS:
· High School Diploma or equivalent preferred.

Duties and Responsibilities:

<li>The Health &amp; Wellness Manager reports to the Center Director and is a salaried, exempt position. </li>
<li>The Health &amp; Wellness Manager is a Registered Nurse that is responsible for the overall supervision of the health and wellness department and services.</li>
<li>The Health &amp; Wellness Manager provides health and wellness assessments and screening, nursing interventions, emergency care, education, and referral services in conjunction with health and wellness staff and health subcontractors.</li>
<li>Orders and maintains current dated pharmaceuticals and clinic supplies. Ensures that assigned staff perform monthly inventories of supplies to include expiration dates. Monitors and ensures the proper storage and condition of supplies and specimens.</li>

Qualifications:
