$37 an Hour
We are currently seeking a dedicated and experienced MDS Coordinator to join our team at Elkader Care Center located in Elkader, IA. The MDS Coordinator is a vital member of our interdisciplinary team, responsible for assessing residents and developing care plans to ensure they receive the highest quality care.
Responsibilities:
<ul>
<li>Coordinate the development and completion of the MDS for each resident</li>
<li>Collaborate with healthcare providers to develop comprehensive care plans</li>
<li>Ensure accurate and timely documentation of resident assessments and care plans</li>
<li>Maintain compliance with state and federal regulations regarding MDS assessments</li>
<li>Participate in interdisciplinary meetings to discuss resident progress and adjust care plans as needed</li>
</ul>
Qualifications:
<ul>
<li>Active RN license in the state of Iowa</li>
<li>Prefer 2 years experience as an MDS Coordinator in a long-term care setting</li>
<li>Strong attention to detail and organizational skills</li>
<li>Excellent communication and interpersonal skills</li>
<li>Ability to work collaboratively in a team environment</li>
