Essential Functions:

<li>Identify and engage health home care coordination patients at Rimrock who have at least two chronic health conditions, such as “asthma, diabetes, heart disease, obesity, mental condition, and substance abuse disorder or one chronic condition and be at risk for another; or one serious and persistent mental health condition.</li>
<li>Assist new patients in completing paperwork and releases of information needed to transfer medical care.</li>
<li>Act as the system navigator and point of contact for patients and families. </li>
<li>Assist patients and their families in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).</li>

CSA Homecare is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to provide one-on-one care for a high-acuity veteran in the Rabun County (30552) area. This is a long-term care position supporting a client who requires tracheostomy care, wound care, and dedicated clinical attention.
We are hiring for 58 total hours/week and are looking for LPNs available to work 30+ hours/week.
Responsibilities:

Provide direct nursing care for a high-acuity veteran in a homecare setting

Perform tracheostomy care and maintenance
</li>

Conduct wound care and monitoring
</li>

Document care provided and communicate with the care team

Requirements:

Valid LPN license in the state of Georgia

Experience with trach care and wound care (Required)
