Responsibilities:
<ul>
<li>Conduct comprehensive assessments of patients' health status and develop individualized care plans.</li>
<li>Administer medications, perform treatments, and provide wound care as prescribed by physicians.</li>
<li>Educate patients and their families on disease management, treatment plans, and self-care techniques.</li>
<li>Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.</li>
<li>Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.</li>
<li>Document patient care accurately and maintain confidentiality of patient information.</li>
<li>Provide emotional support and counseling to patients and their families to promote well-being and coping strategies.</li>
<li>Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.</li>

Qualifications:
<ul>
<li>Current Registered Nurse (RN) license in the state of CA.</li>
<li>1-2 years of clinical nursing experience, preferably in Home Health or Hospice; or a related setting.</li>
<li>BLS/CPR certification required; additional certifications such as ACLS or wound care certification preferred.</li>
<li>Valid CA Driver’s license</li>
</ul>
Documentary Requirements:

<li>Mandatory Physical (within the last 6 months prior to the orientation date). You may bring a note from your physician prior to the Orientation Date</li>

<li>MMR immunization or titer showing immunity prior to your Orientation Date</li>
