Position Summary
This position is based in Adams/Denver County and requires traveling to client homes and within communities. The Care Manager provides direct behavioral health and case management services to adults and families with complex needs, including individuals served through ACT-informed practices. This position will also provide outpatient competency restoration education for individuals that have been deemed incompetent by the court.
 
Schedule

Key Responsibilities
Behavioral Health
<ul>
<li>Conduct brief initial behavioral health screenings to determine eligibility and treatment needs.</li>
<li>Crisis support: Offer immediate, non-clinical de-escalation, ensure safety, and coordinate with clinical team or crisis services.</li>
<li>Psychoeducational groups: Lead or co-facilitate structured groups focused on skill-building (e.g., coping skills, communication, goal setting), based on approved curriculum.</li>
<li>Substance use interventions: Co-facilitate or lead non-clinical groups and individual client support focused on education, relapse prevention, or targeted care management support under supervision.</li>
<li>Collaborate in a shared caseload model to ensure consistent care for clients across the team.</li>
<li>Support clients in developing and achieving treatment goals using motivational interviewing and strengths-based practices.</li>
<li>Offer assistance with activities of daily living, skill building, and developing home routines that foster independence and stability.</li>
<li>Provide individual and/or group Outpatient Restoration Services (Education and Case management) with clients that have been deemed incompetent by the Court. </li>

<li>Apply ACT principles such as assertive engagement, integrated care, and a no-dropout approach.</li>

Housing

<li>Assist clients with navigating housing systems, including completing applications for subsidized housing, transitional housing, or supportive housing programs.</li>
<li>Collaborate with team members to identify barriers to housing, such as credit issues, criminal history, or lack of documentation, and develop plans to address them.</li>
<li>Provide accompaniment to housing appointments or inspections, when necessary, to support client self-advocacy and engagement.</li>

Job Description
The Club Director/Site Coordinator is responsible for the delivery of programs within a designated school Club, leading, managing, directing, and monitoring overall 21st CCLC program operations including safety, high quality programming, driving optimal club experience and service delivery/outcomes for Club Members. The Club Director/Site Coordinator’s duties include staff and volunteer supervision, building relationships with families and the community, and supporting the recruitment and retention of students. Position requires being responsible for various administrative tasks, staff development, providing leadership and supervision.
Key Roles (Essential Responsibilities):
<ul>
<li>Plan and oversee the administration of Club programs and activities that support Youth Development Outcomes and all 21st CCLC objectives.</li>
<li>Establish and maintain Club and 21st CCLC program goals that ensure the health and safety of members. Ensure that Club staff understand and effectively communicate standards of programs; that they ensure program areas are safe, well ventilated, and well lit; and that Club equipment is maintained in good working condition.</li>
<li>Direct members through training and guidance for positive growth and academic achievement, according to their individual needs. Ensure members actively participate in a variety of programs.</li>
<li>Develop, plan, execute and supervise a diversified youth development and academic support program consistent with the goals and objectives of the organization.</li>
<li>Manage 21st CCLC program and activity expenditures within approved budget.</li>
<li>Provide the guidance necessary to maintain discipline and acceptable behaviors in the Club.</li>
<li>Evaluate programs, activities, staff, and volunteers to ensure the appropriateness, quality, and effectiveness of services. Oversee the day-to-day activities in accordance with established standards and goals. Collaborate with Club and school staff to evaluate program efficacy.</li>
<li>Provide leadership to staff for better understanding and needs of the individual member.</li>
<li>Establish and conduct special programs to meet the needs of Club members.</li>
<li>Increase visibility of the Club and the 21st CCLC program via posting of daily schedule, announcements of upcoming events and dissemination of timely information for the development of advertising and promotion through mailings, fliers, and media releases</li>
<li>Promote and stimulate participation in Club and 21st CCLC membership, including the promotion and coordination of inter-departmental activities and special events.</li>
<li>Responsible for recruitment of students from eligible schools to ensure that contracted participation levels are achieved.</li>
<li>Plan, organize and conduct programs utilizing community resources in accordance with all Club and grant guidelines, and organizational policies and procedures.</li>
<li>Maintain accurate records of activities, programs, schedules, and members using computer database/software.</li>
<li>Recommend requisition of supplies and equipment and maintain a safe environment.</li>
<li>Assist in the supervision of the care, maintenance, and cleanliness of equipment, building and grounds.</li>
<li>Manage and maintain facilities and equipment, and ensure productive work environment, maintaining an inventory of all program equipment and keeping them locked and secure during non-program hours.</li>
<li>Administer CPR &amp; first aid as necessary.</li>
</ul>
Relationships:
<ul>
<li>Internal: Maintain close, daily contact with Club staff (professional and volunteer) Club members, and supervisor to receive/provide information, discuss issues, explain guidelines/instructions; instruct; and advise/counsel.</li>
<li>External: Maintain contact with external community groups, schools, members' parents, and others to assist in resolving challenges.</li>
</ul>
Skills &amp; Knowledge Required:
<ul>
<li>Bachelor’s Degree in education, recreation, social work, psychology or a related field or relevant experience.</li>
<li>Master’s degree or masters level course work in education or related field preferred.</li>
<li>Minimum of 5 years’ experience in planning and supervising programs and activities based on the developmental needs of youth.</li>
<li>Knowledge of K-12 public education.</li>
<li>Ability to supervise and motivate students in a fluid environment.</li>
<li>Ability to enforce Boys &amp; Girls Clubs rules and policies in a fair and equitable manner.</li>
<li>Strong leadership skills (management/supervisory experience).</li>
<li>Strong communication skills (verbal, written and listening) and strong conflict resolution skills.</li>
<li>Strong computer skills (Excel, Word).</li>
<li>Strong administrative skills/experience required.</li>
<li>Certificate of first aid and CPR (or willing to receive training and certification).</li>
<li>Valid Florida driver’s license – must be able to meet insurance requirements.</li>
<li>Bi Lingual - (Haitian Creole)</li>
