Qualifications

<li>Graduate of an accredited nursing school with a current RN state license.</li>
<li>5+ years of experience in home infusion and infusion centers.</li>
<li>3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.</li>
<li>Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred</li>
<li>Effective interpersonal, time management, and organizational skills.</li>

The Patient Care Guide (PCG) serves as an integral part of the team, creating a supportivepatient experience and ensuring patient safety. <li>Administer and manage medications for mental health and substance use disorders, including long-acting injectables and Medication-Assisted Treatment monitor for side effects and treatment effectiveness.</li>
<li>Facilitate psychoeducational and therapeutic groups focused on health, wellness, recovery, and harm reduction.</li>
<li>Provide education to clients and families on medications, chronic health conditions, self-care strategies, and wellness planning.</li>
<li>Collaborate with psychiatric providers, primary care clinicians, mental health professionals, and substance use treatment specialists to ensure integrated, coordinated care.</li>
<li>Participate in daily team meetings and contribute to individualized treatment planning.</li>
<li>Conduct home visits and community outreach to support client engagement, medication adherence, and continuity of care.</li>
<li>Maintain accurate, timely, and compliant documentation in accordance with agency policies and regulatory standards.</li>
<li>Support crisis intervention, de-escalation, and stabilization efforts in community settings.</li>
<li>Promote trauma-informed, recovery-oriented, and culturally responsive care practices.</li>
<li>Coordinate with pharmacies and complete prior authorizations for medication approvals; assist providers with Prescription Drug Monitoring Program (PDMP) compliance.</li>
<li>Under the direction of a medical provider, enter laboratory orders, track test completion, and follow up to ensure results are reviewed and addressed.</li>
<li>Assist with provider scheduling and triage client needs to appropriate team members.</li>
<li>Conduct mental health screenings (e.g., PHQ-9, GAD-7, PCL-5) and report abnormal findings to medical providers.</li>
<li>Perform basic health screenings, including vital signs, urine drug screens, pregnancy tests, and other point-of-care testing.</li>
<li>After completing Colorado-specific training, be prepared and willing to initiate an M1 hold when necessary to ensure client safety.</li>
<li>Perform other duties as assigned to support team operations and client care.</li>
</ul>
 
Qualifications
 
Required:
<ul>
<li>Active Registered Nurse (RN) license in the state of Colorado</li>
<li>Valid drivers license and access to reliable transportation</li>
<li>Current CPR certification</li>
<li>Up-to-date vaccinations in accordance with agency and public health guidelines</li>
<li>A minimum of one to two years of experience working with individuals diagnosed with serious and persistent mental illness.</li>
</ul>
 
Preferred:
<ul>
<li>Bachelor of Science in Nursing (BSN)</li>
<li>Experience in community-based, psychiatric, or integrated care settings</li>
<li>Familiarity with the Assertive Community Treatment model and trauma-informed care principles</li>
<li>National certifications such as Psychiatric-Mental Health Nursing (PMH-BC) or Certified Addictions Registered Nurse (CARN)</li>

Total Compensation:
Salary: $82,576-$93,600 annually
