The primary role of the Nursing Assistant (CNA) is to provide assigned residents with routine daily nursing care in accordance with established facility nursing care procedures and, in a manner, compliant with all state and federal regulations as well as with any applicable facility policies and procedures.
QUALIFICATIONS:

<li>High school graduate or equivalent.</li>
<li>Must be a Certified Nursing Assistant in the state of CO.</li>
<li>Ability to safely perform the essential functions of the job without harm to self or others.</li>

ESSENTIAL FUNCTIONS:

<li>Assists residents with personal cares, including bathing, dressing, grooming, oral hygiene, bed mobility, toileting and eating. Documents ADL’S appropriately.</li>
<li>Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc. Understands/supports Resident Bill of Rights.</li>
<li>Serves meals and assists residents with eating. Records accurate food and fluid intake as assigned. Reports any chewing or swallowing difficulty to the nurse.</li>
<li>Assists the resident with the restorative nursing program including but not limited to, passive and active range of motion, ambulation with appropriate assistive devices and use of transfer belt.</li>
<li>Uses safety standards at all times in transfer gait of residents: (a) Uses 2 trained staff members for all mechanical lifts, gait belt and/or proper mechanical equipment for all transfers/ambulation of residents. (b) Uses proper body mechanics. (c) Ambulates/ transfers residents per plan of care. (d) Positions residents to maintain body alignment per policy and procedure and plan of care.</li>
<li>Reports promptly to the nurse any changes in the resident condition.</li>
<li>Answers call lights promptly and determines residents’ needs.</li>
<li>Neatly makes and changes bed linens.</li>
<li>Ensures that residents are clean, neatly dressed according to preference or age, and well-groomed.</li>
<li>Reports any indication of resident illness to the nurse. Completes Stop ’N’ Watch forms on Point of Click or uses paper form.</li>
<li>Follows standard precautions and works within OSHA guidelines. Follows infection prevention and infection control policy/procedure: (a) Follows resident skin care protocol. (b) Properly cleans and stores resident equipment. (c) Maintains sanitary conditions in common areas (e.g., tub rooms, dining areas, and hallways). (d) Follows basic principles of linen handling. (e) Follows principles of universal precautions. (f) Follows isolation precautions. (g) Reports any concern related to infection control to nurse.</li>
<li>Observe and report any noted changes in mental status to the nurse.</li>
<li>Report to charge nurse all incidents/accidents that occur involving employee, residents, and visitors.</li>
<li>Treats and ensures that residents are treated with respect and dignity. Shows respect for resident’s right to confidentiality, dignity and privacy.</li>
<li>Immediately reports any suspected abuse or neglect per facility policy.</li>
<li>Remains aware and takes special precautions to ensure a clean and safe environment for residents and staff.</li>
<li>Reports any equipment malfunction to designated employee.</li>
<li>Completes miscellaneous tasks related to care of residents (e.g., transporting patients).</li>
<li>Reports to nurse prior to shift and at end of shift.</li>
<li>Accurately documents vital signs, weights, intake and output as delegated per facility protocol.</li>
<li>Utilizes electronic systems of documentation as required per facility policy.</li>
<li>Attends designated nursing meetings as scheduled.</li>
<li>Develops and maintains a good working relationship with inter-departmental personnel.</li>
<li>Attends educational offerings and obtains at least twelve (12) hours of continuing education a year per state regulations.</li>
<li>Follows federal and state rules and regulations governing nursing facilities.</li>
<li>Reports emergency situations immediately to nurse.</li>
<li>Responds immediately to an emergency. This includes assisting with the evacuation of residents.</li>
<li>Establishes and maintains respectful communication with all employees, residents and families.</li>
<li>Attends required in-services and completes assigned on-line modules.</li>
<li>Uses proper chain of command to communicate concerns.</li>
<li>Complete all assigned in-services and on-line modules.</li>
<li>Serves on, participates in and attends various committees and meetings of the facility to enhance interdisciplinary process.</li>

Job DescriptionINTRODUCTION
Under the supervision of the Director of Behavioral Health, the Behavioral Health Coordinator is responsible for assuring patients with chronic and or acute behavioral health needs are connected with appropriate health care and social services upon returning to the community. This position is co-located within DC Department of Corrections, as well as in one or more of Unity’s Health Care Clinics within the Community.
MAJOR DUTIES/ESSENTIAL FUNCTIONS
<ul>
<li>Liaises with the Psychiatric, Mental Health and OTP/SUD teams within DC Corrections to identify patients with chronic or acute behavioral health needs requiring intensive discharge planning services.</li>
<li>Conducts a discharge planning interview with each patient prior to release based on the patient release list generated daily.</li>
<li>Liaises with Unity Health Care clinics to admit patients for services including medical-psychiatric-social services following release from DC Corrections.</li>
<li>Conducts a needs interview of each patient referred and establishes a brief action plan.</li>
<li>Assists and applies for Identification cards, health insurance, ADAP (when appropriate), and other emergency assistance programs for the patient at the time of release.</li>
<li>In collaboration with the DBH liaisons at DOC, links patients in need of Psychiatric services with one of the Core Services Agencies operating under DC Department of Behavioral Health.</li>
<li>Links patients in need of Substance Abuse Services with a Unity MAT clinic or other Provider within the DBH provider network.</li>
<li>Maintains electronic copy of services when provided.</li>
<li>Tracks and maintains statistical data on the population served as required.</li>
<li>Participates in monthly team meetings and supervision as required.</li>
<li>Liaises with DBH liaisons co-located within the Department of Corrections (CDF and CTF) as well as staff at the READY Center, and interagency resource center hosted by DOC.</li>
<li>Performs full range of MHC duties at DOC approximately 20% of the time, including covering intakes, assessing suicide risk, conducting safe cell follow-ups, and responding to sick call requests.</li>
</ul>
QUALIFICATIONS
<ul>
<li>Minimum of 3-5 years of prior experience in Case Management.</li>
<li>Minimum of 3-5 years staff in a mental health, shelter, or supportive housing work environment</li>
<li>Minimum of 3-5 years of work experience within the DBH provider network (i.e., either at DBH itself in the forensic or crisis services division, at a DBH-certified Core Services Agency, or at one of the contract MH Crisis Residential Programs operated for DBH), work environment</li>

SUPERVISORY CONTROLS
This position reports to Director of Behavioral Health within the DC Corrections Health Care Delivery System.
GUIDELINES
The BH Coordinator abides by all rules and regulations set forth by applicable licensing and regulatory bodies; and the UHC policies and procedures. 
PERSONAL CONTACTS
The position of BH Coordinator requires contact with staff at all levels throughout the organization. There are also external organizational relationships that may be part of the work of this individual.

WORK ENVIRONMENT

Duties:
