Essential Functions &amp; Responsibilities:

<li>Performs the comprehensive assessment and develops/implements and evaluates the plan of care as identified on admission and subsequent home visits.</li>
<li>Provides appropriate and through education to patients and their caregivers as it relates to the plan of care.</li>
<li>When assigned, serves as case manager to coordinate multidisciplinary services per the plan of care.</li>
<li>Maintains acceptable productivity standard as determined by agency leadership.</li>
<li>Participates in multi-disciplinary team meetings in order to share information and assist with patient management.</li>
<li>Participates in staff and administrative meetings.</li>
<li>Participates in quality and performance improvement activities.</li>
<li>Perform other duties as assigned.</li>
<li>Performs essential functions and responsibilities of the role at Mary Washington Hospital and Stafford Hospital locations, as scheduled. </li>

Qualifications:
<ul>
<li>Graduate of an accredited nursing program and has experience as a Registered Nurse</li>
<li>Valid RN License from Virginia or reciprocal compact state required.</li>
<li>BSN is preferred.</li>
<li>AHA BLS Provider CPR required at hire</li>
<li>NIHSS (Bluecloud) required at hire.</li>
<li>Additional certifications will be required according to the assigned unit</li>
<li>2 years of recent RN acute care experience required</li>
