BAYADA Home Health Care is looking for a compassionate and dedicated full-time, part-time, or per-diem Licensed Practical Nurse (LPN) to join our team. This office services our adult clients on a per visit basis in homes throughout Luzerne and Lackawanna County. 
One year prior clinical experience as a licensed Practical Nurse is required. 
As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:
<ul>
<li>Making home visits to clients living in designated territories.</li>
<li>Performing assigned duties, including administration of medication, wound care, treatments, and procedures.</li>
<li>Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.</li>
<li>Following up with, executing, and properly documenting doctors' orders.</li>
<li>Performing client assessments as necessary.</li>
<li>Case management and coordination.</li>
<li>Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.</li>
</ul>
Qualifications of a Licensed Practical Nurse (LPN):
<ul>
<li>A current license as a Licensed Practice Nurse in the state of Pennsylvania.</li>
<li>A minimum of one year of recent, verifiable experience as an LPN.</li>

BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant, PTA, for a full-timeopportunity performing home health visits for ouroffice.This office services adult and geriatric patients on a per visit basis in territories throughout McLean, VA. 
As a home care Physical Therapist Assistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.

Each Physical Therapist Assistant (PTA) must:
<ul>
<li>Have current licensure or certification in the state of VA</li>
<li>Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or</li>
<li>Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977</li>
<li>Have at least one year’s work experience under the supervision of a qualified Physical Therapist.</li>
<li>Other activities, as requested.</li>
