Shift Structure
<ul>
<li>Shifts are flexible depending on physicians' schedule;</li>
<li>Typically, medical directors work a minimum of 2-3 days/week at each facility they oversee;</li>
<li>Rounding times are flexible with no set start and end time. Preferably, physicians start in the morning between 7-10 a.m. and finish their day when their caseload has been seen;</li>
<li>No nights, weekends, or on-call</li>
<li>Call is covered by Theoria's virtual care team</li>
<li>This is an in-person position</li>

Compensation and Benefits

<li>Compensation ranges approximately from $50,000-$400,000/year depending upon employment status and availability (PRN, Part-Time, Full-Time)</li>

Job DescriptionAbout This Registered Nurse (RN) Case Manager Position
HopeHealth Hospice &amp; Palliative Care is seeking an experienced RN to care for hospice patients in LTC's in the southern part of Rhode Island 
The RN Case Manager is an advocate, negotiator, and leader for the clinical oversight of the patient.
The successful candidate is passionate about providing the best possible care at end-of-life and acts with a sense of urgency and a disciplined approach for the accuracy and quality of the care delivered. 
This is a full-time, 40 hours per week, position with 8am-4:30pm hours. There is a rotating weekend commitment of every 3rd-4th weekend.
 
 
What will you do as a Registered Nurse (RN)?
<ul>
<li>Conducts initial and ongoing assessment of patients </li>
<li>Collaborates with physician to manage patient's plan of care </li>
<li>Evaluates plan of care on a regular basis, and as necessary, to meet the changing needs of the patients </li>
<li>Provides ongoing training and education to patients and family members </li>
<li>Initiates appropriate preventive and rehabilitative nursing procedures, making necessary referrals to community services </li>
<li>Prepares point of care clinical documentation that demonstrates progress toward established goals </li>
<li>Coordinates services, informing the physician, management, and other staff of changes in the patient's condition and needs. </li>
<li>Determines scope and frequency of services needed based on acuity and patient needs </li>
<li>Completes, maintains and submits accurate, relevant and timely documentation regarding patient's condition and care given </li>
</ul>
 
What are the qualifications to be a Registered Nurse (RN) Case Manager?

<li>Graduate of an accredited nursing school; RN required; BSN preferred </li>
<li>Licensed as a Registered Nurse in Rhode Island and Massachusetts preferred  </li>
