Position Summary 
Qualified Nurse Practitioners and Physician Assistants interested in this position will have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs and PAs to connect with patients on a more personal and effective level. Nurse Practitioners and Physician Assistants performing HRAs are not required to treat or prescribe in the home.  
Nurse Practitioner/Physician Assistant Responsibilities 
<ul>
<li>Travel to and from patients’ homes. </li>
<li>Perform comprehensive, non-invasive physical assessments. </li>
<li>Review medical history and ensure accurate and timely documentation. </li>
<li>Conduct medical reconciliation to ensure patient safety and compliance. </li>
<li>Provide basic patient education on wellness and preventative healthcare measures. </li>
</ul>
Nurse Practitioner/Physician Assistant Qualifications and Experience Requirements:  
<ul>
<li>Master’s Degree OR commensurate experience and satisfactory completion of NP/PA license </li>
<li>Active State NP/PA license </li>
<li>Active National board certification </li>
<li>Current BLS certification </li>
<li>12 months of experience as a paid advanced clinical provider is preferred but not required </li>

Position Description
Responsible for ensuring the delivery of efficient and effective nursing care while achieving positive clinical outcomes and resident/family satisfaction in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Operates within the scope of practice defined by the state Nurse Practice Act. Responsible for resident care and direction of nursing care during assigned shift; includes staff assignments, mentoring and educating nursing personnel, working with physicians and other medical professionals.

Responsibilities:
<ul>
<li>Effectively carries out medication management to ensure adequate supplies and that all medications are handled in accordance with company policy.</li>
<li>Ensures that residents attending physician and family or responsible party are promptly notified of any significant change in the resident's health condition.</li>
<li>Ensures complete and prompt reporting of incidents, accidents, and adverse events to Administrator and Director of Nursing.&nbsp;</li>
<li>Ensure that residents’ accidents/incident, adverse event and grievances/concerns are fully documented, investigated, reported and addressed in accordance with company policies and procedures and the Federal/State rules and regulations.</li>
<li>Understands, upholds and promotes the rights of the residents.</li>
<li>Ensures residents can exercise rights without interference, coercion, discrimination, or reprisal from the facility.</li>
<li>Ensures protected health information is kept confidential.</li>
</ul>
Requirements:
<ul>
<li>Graduate of an approved Registered Nurse program and licensed in the state of practice, required.</li>
<li>Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.</li>
<li>Current BLS/CPR certification.</li>
</ul>
