We are looking for an experienced NP or PA with a background in Orthopedics, Emergency Medicine and/or Urgent Care for this dynamic independent practice model position for Essentia Health. 
Practice Specifics:

<li>Advanced Practitioners will comprise the core staff, with MD oversight from orthopedics.</li>
<li>Free standing location away from the main campus.&nbsp; 3 exam rooms, a cast room in addition to state-of-the-art Radiology Suite.</li>
<li>VERY independent practice model with support from onsite radiology tech and athletic trainer</li>
<li>No follow-up care – Initial workup and medical care only, all follow up care done by referral</li>
<li>Work shifts will be (3) twelve-hour days for a full-time employee or a variation of these hours.</li>
<li>Staffing model is:&nbsp;

<li>Monday – Friday 12:00 p.m. – 8:00 p.m.</li>
<li>Saturday 11:00 a.m. – 4:00 p.m.</li>

o Monitor and manage acute GI conditions and provide pre- and post-procedure care.
o Coordinate care transitions between hospital and clinic settings.
Anticipated Schedule:
Monday – Friday (Day Shift)
· Clinic – 36 patient contact hours/week
· Hospital – 40 patient contact hours/week
Rotating Weekend Coverage
EDUCATION:
1. Master’s or Doctorate degree in Nursing or Bachelor’s or Master’s degree in Physician Assistant Studies from an accredited program.
LICENSURE/CERTIFICATIONS REQUIRED:
1. Current licensure as a registered nurse in Minnesota and Wisconsin.
2. Current licensure as a Nurse Practitioner or Physician Assistant in Minnesota &amp; Wisconsin (WI would need to be obtained within 3 months of hire).
3. Current certification in CPR.
4. National certification, in applicable area.
5. Successful completion of Essentia Health credentialing process prior to practice.

<li>Call will be 1:6. Casual staff used to cover call on an as needed basis</li>
<li>Full scope of Nephrology practice</li>
<li>Clinical Nephrology, hypertension and post-transplant care</li>
<li>Hospital practice includes ICU and acute dialysis services as well as apheresis, home hemodialysis, Plasmapheresis, Continuous Renal Replacement, CAPD and CCPD</li>
<li>Dialysis population of around 300 patients with 2 units in Duluth (18 &amp; 22 stations) and 7 satellite locations (some have home dialysis). Dialysis population of around 300 patients with 4 Twin Ports locations and&nbsp; 5 satellite locations.&nbsp;</li>
<li>Medical directorships are associated with out-patient dialysis units. The Sites are in these communities:&nbsp;Duluth, Hermantown, Virginia, Grand Rapids, Hibbing, Cloquet, MN and Superior, Ashland and Hayward, WI.&nbsp;</li>
<li>Inpatient care at SMDC Medical Center and St. Mary’s Medical Center, a combined 500 bed tertiary care hospital complex</li>
