The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Mission for Sunrise Senior Living, “to champion the quality of life for all seniors” in accordance with federal, state/provincial, and local laws, standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents.

Medication Administration:

<li>Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.</li>
<li>Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.</li>
<li>Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse).</li>
<li>Ensures that medications are passed according to times.</li>
<li>Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses.</li>
<li>Maintains confidentiality of all resident information including resident medication among other residents.</li>
<li>Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.</li>
<li>Restocks medication cart after all medication passes.</li>
<li>Assists in checking medication regardless of packaging system.</li>
<li>Assesses the residents to determine need for “as needed medication” and appropriately documents and reports to supervisor.</li>
<li>Counts all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift.</li>
<li>Maintains and cleans the Medication Room, medication carts and treatment carts for neatness, cleanliness, availability of medications, and expired medications.</li>
<li>Follows re-fill process for medications.</li>
<li>Helps residents maintain independence and promotes dignity and physical safety of each resident adhering to the Sunrise Principles of Service.</li>
<li>Strives to understand and respond to each resident with empathy, always remaining mindful of the resident’s unique communication patterns, history, and basic human needs.</li>
<li>Practices routinely good standard care precautions of cleanliness, hygiene, and health .</li>
<li>Audits medication carts.</li>
</ul>
Resident Care:
<ul>
<li>Notifies RCD of any resident and/or family concerns through the LEAD process.</li>
<li>Attends and actively participates in daily Cross Over meetings facilitated by the LCM.</li>
<li>Conducts Service and Health Updates as directed by RCD.</li>
<li>Participates in the development of the Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.</li>
<li>Transcribes orders.</li>
</ul>
Risk Management and General Safety:
<ul>
<li>Partners with community team to ensure community is in compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.</li>
<li>Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.</li>
<li>Reports all accidents/incidents immediately.</li>
<li>Reports all unsafe and hazardous conditions/equipment immediately.</li>
<li>Ensures any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.</li>
<li>Complies with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.</li>
<li>Understands and practices the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.</li>
<li>Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.</li>
<li>Ensures oxygen tanks are stored safely, exchanges guest/resident’s tanks when empty and monitors to make sure liters of oxygen are at prescribed levels.</li>

The Care Giver/Med Tech role includes providing hands on care, physical and emotional support to each resident while maintaining a safe and comfortable home like environment.
Responsibilities:
Care Giver:
<ul>
<li>Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift.</li>
<li>Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities.</li>
<li>Responds to security system and resident call bells promptly.</li>
<li>Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs.</li>
<li>Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms.</li>
<li>Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.</li>
<li>Maintains cleanliness of resident’s room and work areas.</li>
<li>Practices good standard care precautions of cleanliness, hygiene and health.</li>
<li>Helps residents maintain independence, promotes dignity and physical safety of each resident.</li>
<li>Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed.</li>
<li>Engages residents in life skills and other life enrichment activities.</li>
<li>Greets and assists all internal and external customers, guests, family members, residents, vendors and team members.</li>

Medication Tech duties:
<ul>
<li>Reviews service plan to learn pertinent information about residents.</li>
<li>Assists/observes medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass.</li>
<li>Documents and initials form as medications are given including appropriate documentation for refusal or missed doses.</li>
<li>Maintains confidentiality of all resident information including resident medication.</li>
<li>Reports all resident concerns made while assisting resident with the medication to the Nurse and/or Health Care Coordinator (HCC).</li>
<li>Restocks medication cart after all medication passes.</li>
<li>Assists in checking medication regardless of packaging system.</li>
<li>Counts all narcotics with another Medication Care Manager or Nurse each shift.</li>
<li>Maintains and cleans the Medication Room, med charts, treatment carts for neatness cleanliness, availability of medications and expired medications.</li>
<li>Follows re-fill process for medications.</li>
<li>Participates in the development of the Service Plan and monthly updates.</li>
<li>Takes and records temperature, blood pressure, weight, pulse and respiration rates.</li>
<li>Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.</li>
<li>Other duties as assigned by the Supervisor.</li>

Qualifications:

POSITION SUMMARY: 
The Facility Safety Manager oversees the Houston Food Bank’s (HFB) health, safety, environment, and emergency operations. This role provides directional leadership for standards for facility safety, as well as develops and manages: 1) (OSHA) Compliance programs, 2) First Responder programs, 3) Emergency Response Plans and drills, and 4) related Safety Orientation Programs. This role develops and maintains working relationships and lines of communication with local and state emergency management personnel and other organizations involved in disaster relief and recovery for HFB.

ESSENTIAL JOB FUNCTIONS:

<li>Following a disaster, facilitates after-action sessions and reports.</li>

SUPERVISORY RESPONSIBILITIES: 
May manage one (1) or more director reports and indirectly provide guidance and/or oversight to employees and/or contractors. Carries out responsibilities in accordance with the organization's policies and applicable laws. Develops and maintains roles, responsibilities, and expectations. Practice performance management in support of associate development, and perform administrative duties associated with being a manager of associates. Responsibilities include interviewing contractors, planning, assigning, and directing work, and resolving problems. QUALIFICATIONS: 
Education/Experience:
