DESCRIPTION: Counsel individuals and groups on nutritional practices designed to prevent/manage disease and promote good health. Evaluates individual needs; develop nutritional care plans; instruct individuals and their families; performs nutrition screenings; and offers advice on diet-related concerns such as weight loss, cholesterol, diabetes and other chronic diseases/conditions.
 
SUPERVISION: Supervised by Chief of Clinical Services for operations and supervised by Registered Dietitian for clinical purposes.
 
TYPICAL PHYSICAL DEMANDS: Requires prolonged sitting, standing, long hours, lifting up to 25 pounds. Travel required. 
 
FUNCTIONS AND RESPONSIBILITIES:

<li>Assess nutritional needs, diet restrictions and current health plans, to develop and implement dietary-care plans and provide nutritional counseling.</li>
<li>Consult with physicians and health care personnel to determine nutritional needs and diet restrictions of patient.</li>
<li>Advise patients and their families on nutritional principles, dietary plans and diet modifications, and food selection and preparation.</li>
<li>Counsel individuals and groups on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life.</li>
<li>Develop policies (related to diet and nutrition) for disease management programs to assist in health promotion and disease control.</li>
<li>Develop curriculum and prepare manuals, visual aids, course outlines, and other materials used in teaching.</li>
<li>Coordinate diet counseling services.</li>
<li>Plan and conduct training programs in dietetics, and nutrition, health-care personnel, health promoters and the general public. </li>
<li>Function as part of a team while being self-motivated and self-directed.</li>
<li>Maintains strict confidentiality of patient information and patient records.</li>
<li>Serves as resource and nutrition specialist for the behavioral health program and the Diabetes Self-Management Education Program staff.</li>
<li>Supports CMO in program development, implementation and oversight. </li>
<li>Able to successfully interact with people from diverse backgrounds and occupations. </li>
<li>Travels to all four clinic sites as requested. </li>
<li>Maintains strict confidentiality of patient information and patient records.</li>

MINIMUM QUALIFICATIONS:

<li>Bachelor's degree in nutrition, health sciences or related field required. </li>
<li>Three years' experience in nutritional education and disease management teaching/counseling.</li>
<li>Active affiliation with appropriate Dietetic networks and organizations, e.g. American Dietetic Association (ADA), Texas Dietetic Association, etc. </li>
<li>Must have a current BLS certificate.</li>
<li>Bilingual in English and Spanish preferred.</li>

I am a family physician with a medically supervised weight loss program and am looking to bring on a dietitian/nutritionist to help manage the diet portion of the program. Our program offers an initial three-month plan with two different tiers of service. All clients undergo an initial intake with our medical providers, which includes a review of medical history and bloodwork. The medical provider will determine if weight loss medication is appropriate.
The dietitian will be responsible for getting a comprehensive history of eating habits, weight struggles, previous diets, successes and failures. The dietitian will then individually tailor a diet plan for each client. Clients will have a minimum of monthly visits with the dietitian for the lower tier, up to weekly visits for the higher tier to review progress, provide feedback, and offer guidance.
The position is expected to be 10-15 hours per week and could lead to full-time employment as the program grows. It will involve a combination of in-office appointments and remote telemedicine appointments,

As a Nutritionist, you will provide clinical and nutritional assessment, education, counseling, and services to participants in the Women, Infants, and Children Supplemental Food Program (WIC). Maintain patient/client confidentiality and ensures compliance HIPAA and WIC policy and procedures.

ESSENTIAL DUTIES &amp; RESPONSIBILITIES
<ul>
<li>Screens, assesses, and evaluates client’s nutritional status during client interview. </li>
</ul>
-Determines eligibility of clients for WIC Program based on nutritional status.
-Performs heights, weight measurements, as well as hemoglobin.
-Documents summary of dietary assessment, client goals and plans in client record. 
-May be required to screen applicants for income eligibility in the WIC program.
 Verifies applicant residency, income, identification, and Medicaid status. Completes and 
 reviews client registration information for participant’s electronic WIC record and sets up EBT 
 accounts
-Ensures certification documents and other records are accurate and complete prior to submission.
-Maintains documentation in client’s electronic record according to WIC guidelines.
-Issues required notification to ineligible participants.

<li>Provides a Patient Centered nutrition counseling approach and education to WIC clients</li>
</ul>
-Counsels high-risk participants on an individual basis.
-When appropriate, provides diet planning, counseling, and instruction to other WIC clients on individual and group basis at various Clinic locations correlating nutrition needs with general health status, culture patterns, personal preferences, and economic factors. 
<ul>
<li>Promotes breastfeeding to prenatals. Works with postpartum clients who are experiencing difficulties with breastfeeding.</li>
</ul>
-Makes referrals to lactation consultant as needed.
<ul>
<li>Provides referrals to community programs for specialized nutrition services/food assistance and other referrals as needed.</li>
<li>Consults with multi-disciplinary team to assure continuity of care between programs and a comprehensive health care plan.</li>
<li>Participates in follow-up care of clients, including by telephone, when appropriate.</li>
</ul>
-Maintains documentation on all contacts made with WIC clients. 
<ul>
<li>Actively participates in efforts to maintain and/or increase Clinic’s assigned caseload.</li>
<li>Meets or exceeds individual productivity goals on a daily basis.</li>
<li>Participates in health fairs and community outreach as assigned.</li>
<li>Prepares lesson plans, visual aids, bulletin boards, pamphlets, and literature useful for nutrition education/classes.</li>
<li>Checks email on a regular basis to remain current on any changes that may impact nutrition department activities. </li>
<li>Attends and participates in meetings and inservices as required. Participates in professional development activities. Serves on committees as requested. </li>
<li>Performs duties in accordance with NHC Standards of Conduct and Mission Statement. </li>

REQUIRED QUALIFICATIONS:
Education / Training
Bachelor’s or Master’s Degree in Dietetics, Nutrition or Nutrition Sciences, Public Health Nutrition, Community Nutrition, or Clinical Nutrition. 

Experience
A minimum of one year of teaching experience maternal and child health or public health setting preferred.
Licensure/Certification
Prefer Registry Dietitian Eligible (RDE). CPR certification as required by NHC.
Physical Requirements
Prolonged sitting, infrequent to occasional walking or standing. Requires full range of body motion, manual dexterity, and eye-hand coordination sufficient to assist clients and operate office/electronic equipment. Requires corrected vision and hearing to normal range to record, prepare, and communicate appropriate reports. Requires ability to lift and carry items weighting up to 35 lbs. Requires exposure to communicable diseases and bodily fluids. Requires ability to work under stressful conditions. 
Work Environment
Normal clinic environment and offsite in various types of environments. Work may involve irregular hours, including evenings or weekends. Requires traveling outdoors in all types of weather. Involves frequent contact with other staff, physicians’ offices, and clients. Contact may involve dealing with sick, angry, or upset people. Frequent exposure to communicable diseases, toxic substances, and other conditions common to a Clinic environment. OSHA Category I – Tasks involve contact with blood or body substances.
