KNOWLEDGE &amp; SKILLS:

<li>Effective communication and multi-tasking skills in a high-volume, fast-paced, team-oriented environment.</li>
<li>Experience as a RN, Medical Case Manager is ideal, or a clinical background in orthopedics, neurology, or rehabilitation is preferred.</li>

PAY RANGE:
CorVel uses a market based approach to pay and our salary ranges may vary depending on your location. Pay rates are established taking into account the following factors: federal, state, and local minimum wage requirements, the geographic location differential, job-related skills, experience, qualifications, internal employee equity, and market conditions. Our ranges may be modified at any time.
For leveled roles (I, II, III, Senior, Lead, etc.) new hires may be slotted into a different level, either up or down, based on assessment during interview process taking into consideration experience, qualifications, and overall fit for the role. MAJOR FUNCTION:
Under the direct supervision of the Senior Director of Behavioral Health and Supervisor of Behavioral Health provides social work services. The Medical Case Manager provides case management services in an integrated care and trauma informed manner. The Medical Case Manager works in a patient centered and collaborative manner providing services to patients which includes but is not limited to case management, advocacy, HIV-related counseling, identification of mental health or substance use needs as appropriate and follow up services to patients via face to face or through telemedicine. Is responsible to work independently, managing a diverse and sometimes difficult caseload; does related work as required.
 Essential Functions:
Direct Service:

<li>Assesses the presenting and underlying psychosocial needs of all clients in the Project Access program. Composes a thorough social work assessment and treatment plan.</li>

Summary: As part of an interdisciplinary team, the Medical Case Manager provides case management services for People living with HIV/AIDS and their families/loved ones. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.
Essential Duties and Responsibilities: 
<ul>
<li>Perform participant intakes assigned by program manager by conducting detailed, in-person, needs assessment interview(s) and arranging health/medical/dental/mental health assessments as needed.</li>
<li>Complete re-assessment of participant needs every six months or as determined by participant’s needs and the program requirement.</li>
<li>Maintain a caseload of 25-45 active participants with HIV/AIDS that requiring comprehensive medical case management services.</li>
<li>Develop and maintain a therapeutic relationship that encourage the participant active engagement to care. </li>
<li>Develop, document and implement an overall service plan for each participant with periodic follow-up action plans as needed. Document services and accomplishment of the service/action plan objectives. Reassess the participant periodically to determine changes in service needs; adjust the service plan and action plans as required.</li>
<li>Assist the participant in accessing needed services, both from the organization and from other providers, advocating for access and for prompt and competent services where needed and coordinating care with other providers as needed.</li>
<li>Counsel participants on current status of and anticipated changes in their health, emotional, psychological and social condition and family and support relationships, performing crisis intervention where needed.</li>
<li>Assess the needs of the participant's family and support network; help solve problems, enhancing their ability to cope and support.</li>
<li>Ensure that participant is registered with a medical provider, maintain medical appointments as scheduled and is adhering to medical regimen and other medical direction.</li>
<li>Maintain consistent contact with medical providers to ensure receipt of medical updates and to facilitate participant’s adherence to treatment. </li>
<li>Maintain regular contacts with the participants according to the program requirements. </li>
<li>Complete all documentation in a thorough manner consistent with organization’s standards.</li>
<li>Perform outreach to human service providers, potential participants and the community at large to broaden awareness of agency programs and to assist other providers in expanding and tailoring their services.</li>
<li>Participate in the preparation of service reports and evaluations.</li>
<li>Become and remain informed of the extent and quality of relevant services in the Chicago metropolitan area and in the agency service area, and participate in service consortia as requested.</li>
<li>Represent the agency at community events and activities and to other organizations and agencies.</li>
<li>Successful complete the case manager certification training sponsored by the AIDS Foundation of Chicago. </li>
<li>Complete a minimum 12 trainings per grant year as required by the AIDS Foundation of Chicago. </li>
<li>Other duties may be assigned.</li>

Medical Case Manager
Under the direction of the Medical Case Manager Supervisor and HIV Program Director, the Medical Case Manager is responsible for the provision of quality, compassionate, timely and accurate medical case management services to individuals including individuals living with HIV. Provide client centered services that link clients with health care, psychosocial and other services to insure timely coordinated access to medically appropriate levels of health and support.

Core Duties

<li>Conduct client needs assessment and acuity scale to determine immediate needs and linkage to medical and support services.</li>
<li>Provide treatment adherence counseling to ensure that the client is ready for and adheres to HIV treatments.</li>
