DESCRIPTION OF JOB RESPONSIBILITIES AND DUTIES
The Clinic Operations Assistant serves as direct support for all operations tasks within our clinics. This includes all ongoing tasks to keep the clinic running smoothly as well as any necessary tasks to support the administrative functioning of the programs within that clinic. 

PROGRAM:

<li>Meet with program management staff as needed to review assignments and task completion. </li>
<li> Provides general information regarding the services The Hume Center provides, answers routine inquires. </li>
<li> Training and coordinating communication between the team and clients. </li>
<li> Logs referrals on the computer and in the respective binder and maintains referrals. </li>
<li> Copies forms that are assigned according to program and department needs at that location.</li>
<li> Filing according to program and department protocol. </li>

<li> Participate in staff meetings and annual Hume Center events. </li>

JOB SUMMARY
This position reports directly to the Finance Manager and supports the Divisional Managers. This position provides direction and oversees the daily office operations, ensuring the smooth functioning of various financial, administrative and clerical tasks in support of the maintenance of District assets, including but not limited to:
Recreation Facilities (Regional, Villages, and Neighborhood), Postal Facilities, dog parks, archery, and air gun ranges, sports courts, softball complexes, fitness trails and equipment, recreational storage areas, woodshops, District landscaping, irrigation, water transfers, pump stations, District maintained roads and stormwater piping infrastructure, stormwater basins, multi-modal (paths, bridges, and tunnels), fire stations, Town Center infrastructure, District office space, decorative water features, sidewalks, curbs, signs, pavement markings, fountains, village and villa entries and sign walls, perimeter walls and fencing, gatehouses, and gates including automated gates, lighting, security cameras, wetlands and wetland flood control structures, preserves (gopher tortoise, burrowing owls, eagles, and kestrels), lined basins (liner repairs and maintenance), ground depressions / geological anomalies and nuisance alligator management.
This is a multi-faceted position responsible for working with residents, customers, contractors and members of the business community. Establishment of daily communication and a mutually productive working relationship between other departments and contractors is essential.
ESSENTIAL DUTIES AND RESPONSIBILITIES (Includes, but not limited to) 
General/Clerical

<li>Coordinates procedures for systematic retention, protection, retrieval, transfer and disposal of records.</li>
<li>Schedules, prepares agendas, attends, takes and distributes meeting minutes for all assigned Department personnel.</li>
<li>Coordinates and attends all contractor meetings at designed recreation centers.</li>
<li>Schedules and maintains all calendar appointments for assigned District personnel.</li>

Project Management

<li>Maintains contract files, providing information to Contract Manager regarding renewals, amendments, etc. as requested.</li>
<li>Accurately prepares purchase requisitions or change orders within required timeframes for the timely creation of purchase orders.</li>
<li>Regularly follows up with assigned staff to ensure purchase orders have been received for all requisitions that have been processed.</li>
<li>Codes Bank of America P-Cards for assigned Department personnel.</li>
<li>Adds all necessary information to the turnover documents and sends them to the District Utility Billing Department.</li>
<li>Update and create Contract amendment sheets for Purchasing Department.</li>
<li>Contacts DPM Financial Coordinator for coding accuracy and budget funding</li>
<li>Sends quarterly reports to the Finance Department for Right of Way maintenance agreements.</li>
<li>Accurately codes invoices for processing, forwarding to assigned District staff for approval within required timeframes.</li>
<li>Creates and accurately maintains billing sheets for vendors.</li>
<li>Keeps apprised of new contracts and addendums on a regular basis and ensures correct amounts are being paid on invoicing.</li>
<li>Verifies all contractor’s insurance is up to date.</li>
<li>Assists in annual budget preparation &amp; documentation.</li>

•Will Perform Work Performance Testing in all NPRC departments along with FRA efficiency testing.

<li> Book field trips and group reservations for both Franklin Park Zoo and Stone Zoo</li>
<li> Create letters, forms and reports pertaining to the office of the Director of Guest Operations</li>
<li> Fill and maintain office equipment such as copy and fax machines to ensure proper working order, as well as troubleshoot issues and coordinate maintenance calls</li>

DEPARTMENT: Lincoln Health Family Practice Clinics

PURPOSE:
Provides customer service and delivers nursing care to patients in a timely and professional manner. 
Reports to: Clinic Administrator for administrative supervision and to the Physicians/Advanced Practice Providers in the Lincoln Health Family Practice Clinics for nursing supervision. 
The Certified Nursing Assistant will work at a Family Practice Clinic that serves a largely rural/frontier population on the Eastern Plains.
DIMENSIONS:
The Certified Nursing Assistant will ensure the efficient and effective flow of patients during scheduled clinics through performing nursing duties. The employee will participate in the development of clinic objectives and participate in quality assessment and improvement processes and activities. The employee will maintain patient care supplies, equipment and the environment.
The Certified Nursing Assistant will protect the dignity, privacy and confidentiality of patients and their families, as well as co-workers and others.
Lincoln Health Family Practice Clinics support and encourage a multidisciplinary team
approach to whole person patient centered care with the integration of primary care and
behavioral health services.
ENVIRONMENT:
Lincoln Health is a 15-bed Critical Access Hospital co-located with 35-bed Long Term Care Unit. The facility employs 150-200 employees. Lincoln Health has 3 Family Practice Clinics, located in Hugo, Limon and Flagler. A Mobile Clinic that provides services all along the Eastern Corridor is also part of Lincoln Health Clinics. 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Acts in a professional manner at all times.
• Works using the guidelines established from the Nurse Practice Act, Lincoln Health standards, policies and procedures and nursing judgment.
• Patient is approached in a kind, gentle and friendly manner. Respect for the patient’s dignity and privacy is consistently provided.
• Interventions are performed in a timely manner. Explanations for delays in responses are provided.
• Safety concerns are identified and appropriate actions are taken to maintain a safe environment.
• Rooms are neat and orderly
• Demonstrate behaviors consistent with facility mission, vision and values
• Maintains professional attire and appearance. Scrubs may be worn and are expected to be laundered and presentable.
• Acknowledges that all patients, family members, visitors, co-workers, employees of LCH and outside providers are customers.
• Provides care and interactions to customers with dignity and respect demonstrating quality customer service.
• Maintains confidentiality and patient rights regarding all patient information.
• Conducts self in a professional manner in compliance with facility policy.
Daily responsibilities
• Sort and file records on a daily basis to include all medical record information received from patient care staff, hospitals, physician offices and other outside sources. 
• Obtain and record health histories
• Complete all documentation timely, efficiently and in its entirety
• Maintain health record and document all items required for Meaningful Use
• Verify all messages have been handled for the day
• Assisting with the front office scheduling, referrals, medical records, and assisting nursing staff in the Clinics.
• Maintains knowledge of equipment set-up, maintenance and use.
• Provides input in the formulation and evaluation of standards of care.
• Ensure tasks, labs, and diagnostics have been reviewed with patients and addressed as appropriate. 
• Initiates emergency support measures (ie. CPR, protects patients from injury).
• Log off their work stations properly
• Provides excellent customer service to his/her team, patients, and public
• Maintains an organized and clean work area
• Emergency situations are recognized and appropriate action is instituted.
• All emergency equipment can be readily located and operated (emergency oxygen supply, drug box, fire extinguisher, etc.)
• Other patient care as assigned (patient recalls, reminders, telephone triage, injections, immunizations, etc. according to clinic policy and procedures).
• Attends all meetings required by CEO and Clinic Administrator 
• Participates in clinic staff meetings
SUPERVISORY RESPONISIBILITIES:
The Certified Nursing Assistant does not have any supervisory requirements. 
QUALIFICATION REQUIREMENTS:

EDUCATION AND/OR EXPERIENCE:

• Successfully attend and pass a CNA program 
• Licensures:

• Relevant Work Experience:
• Experience in providing nursing services in a rural area, primarily family practice clinics is recommended.

• Basic understanding of computer skills: Excel, Word and EHR, as well as basic typing skills. 
LANGUAGE SKILLS:
Ability to read, analyze and interpret financial reports and legal documents. Ability to respond to common inquiries or complains from patients, regulatory agencies or a member of the board. Ability to effectively present information to public groups and/or boards of directors. 
REASONING ABILITY:
• Ability to define problems, collect data, establish facts and draw valid conclusions 
• Participates in the identification of staff educational needs.
• Participates in the development of clinic objectives.
• Participates in the quality assessment and improvement process and activities.
OTHER SKILLS AND ABILITIES:
Ability to use a computer. Knowledge of basic computer functions and electronic health records. Basic knowledge of Microsoft Word and Microsoft Excel.
