SWLA Center for Health Services, Inc. will use the services of mid-level practitioners to compliment the care being provided by the Physicians employed by the Center.  For purposes of this job description, a nurse practitioner is an advanced practice nurse that helps with aspects of patient care, including diagnosis, treatments and consultations.  They will work with the other clinical staff a SWLA only in the ambulatory setting and can perform independently in collaboration with the physician who has certified their collaborative agreement.  Based on HRSA guidelines, Advance Practice Nurses (Nurse Practitioners) are expected to complete appropriate number of patient encounters per year.  They are expected to have a valid Louisiana Nursing license and DEA number.  They must, on an annual basis, obtain the necessary continuing education credits to maintain their license in good standing.  Must maintain certification in Basic Life Support and Advanced Cardiac Life Support. 
EDUCATION, TRAINING AND EXPERIENCE:

<li>Must be willing to travel to and from the service sites as needed by CMO or designee.</li>

Requirements
<ul>
<li>Board certification from a certified Nurse Practitioner or related program which entails training as well as practice in the above mentioned duties. </li>
<li>Current, valid license to practice medicine in the State of California, including DEA license and CPR required.</li>
<li>Two years of experience as a Nurse Practitioner preferred, but not required.</li>
<li>Previous experience providing medical care within a community health setting preferred.</li>

<li>Board Certified in designated specialty field of practice is required. </li>

Medicor Medical Group, Inc. is a Rancho Cucamonga based Primary care clinic looking for a part-time Physician Assistant or Nurse Practitioner who will provide care to patients when they need it most. Applicant must possess all appropriate licensure and certification requirements with excellent written and verbal communication skills. This position will work under the supervision of 1 physician.
 Provide total patient care under the supervision of a physician
 Perform physical examinations and document preventative health measures
 Manage patient treatment plan
 Facilitate referrals, consultations, therapeutic services, etc.
 Assist physician during patient examinations
 Document patient chart and other documents as appropriate
 Orders diagnostic test relative to patients age-specific needs
 Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
 Assists with the provision of care in accordance with facility, state, and federal regulations.
Basic Qualifications: 
Experience
 Minimum one (1) year experience as NP or PA.
Education
 Graduate of an accredited RN and NP Program OR graduate of an accredited PA program.
 Masters Degree in Nursing (effective 1/1/03 for those not previously grandfathered in by CMS- (NP only)).
