A Dietitian, creates nutrition plans to improve the health and physical conditions of patients. Their main duties include analyzing patients' health status, goals and dietary restrictions, tracking clients' dietary intake and progress and educating patients on how to maintain a balanced diet.
They must ensure that proper courtesy is established when serving in-house patients. Knowing who that exact patient is and who's to receive a menu-listed meal is crucial knowledge. The Dietitian is responsible for:

<li>Coming up with a core meal plan for general patients, for special requests or for critical needs</li>
<li>Acting in the role of counselor and thus sustaining patient morale</li>
<li>Maintaining documentation from aides and management as a daily routine</li>
<li>Keeping track of what has or hasn't worked for a patient, which is crucial to providing proper meals later on</li>
<li>Ensuring the cleanliness of a workspace and proper use of utensils during food preparation</li>
<li>No home visit but see home patients in clinic</li>
</ul>
Qualifications:
- Home therapy experience Highly preferred
- Bachelor's degree in nutrition, health, or a related field
- State licenses or additional certifications may be required
- Renal is not required but preferred

Neighborhood Health Association coordinates with and provides nutrition services to the Women, Infant and Children (WIC) supplemental food program clients.. Plans, develops, directs, and evaluates the nutritionalcomponentof the WIC program. 
 
Responsible for certification, rectification, and follow-up:

Skills/Qualifications:

The Renal Dietitian will provide nutrition education, counseling and interventions as appropriate, in the adjustment of both the Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) patients to their disease states and therapies.
This position is Responsible for supporting, understanding, interpreting, and implementing the philosophy and policies of the Facility.
Assists the Director of Nursing by directing the facility towards obtaining results in the following area(s): quality, patient satisfaction, business management, teamwork, culture and employee satisfaction.
Responsibilities:

<li>Supports Facility’s mission, goals and values.</li>
<li>Performs and documents initial nutrition assessments on all newly-admitted ESRD patients; Performs follow up assessments as needed.</li>
<li>Collaborates with physician in determining nutrition needs and diet prescriptions for all CKC and ESRD patients.</li>
<li>Monitors progress of all patients, including at least monthly review of blood chemistries, fluid status, and nutritional status, with interventions as needed.</li>

<li>Participates in orientation of new dietitians.</li>
<li>Attends and participates in interdisciplinary facility patient team meetings, including, but not limited to, quality improvement, development of the patient’s plan of care, and team building.</li>
<li>Assists in coverage of Nutrition Services in cases of LOA, vacations, and census increase.</li>
<li>Meets all State and Federal CMS requirements pertaining to the provision of nutrition services to the CKC and ESRD patient. Supports any CMS/Network studies or data collection activities.</li>
<li>Participates in education programs at the facility and patient level.</li>
<li>Plays a leadership role in the education of and implementation of the quality assessment and performance improvement (QAPI) processes.</li>
<li>Participates in development of and execution of the facility’s operational plan.</li>
<li>Responsible for self professional development and maintenance of necessary credentials.</li>
