Primary Purpose of the Position: 
Carolina’s Creative Counseling, PA provides Outpatient Behavioral Healthcare services to children, adolescents and adults who are in need of clinical treatment. This service is to be provided by licensed professionals. Outpatient Therapy provides intensive clinical therapeutic services to children adolescents, and adults with identified mental health or substance abuse diagnosis. This service is designed to meet the specific mental health needs of the identified client as they relate to their diagnosis and can be provided in-home, in-office, in-school or other approved/appropriate community setting. Outpatient therapy includes psycho-educational activities/interventions designed to assist the client in the reduction of dysfunction. Services are provided with client-centered and family-centered approaches and are delivered through intensive, time-limited, goal specific interventions. 

Description of Duties and Responsibilities: 
<ul>
<li>To deliver in-home or in-office services to clients as requested by the area mental health including the specific hours requested. These services may be provided in the school setting by therapists identified as members of the school based therapy unit if the client’s presenting issues would be better addressed in that setting;</li>
<li>To provide therapeutic interventions to the identified client in order to enhance and foster the client’s optimal functioning and interactions in the home and community;</li>
<li>To continually assess client needs, service availability and appropriateness;</li>

Job Description: 
Constellation Health is looking for a compassionate and empathetic Palliative Care Nurse Practitioner (NP) to work in our palliative/hospice program. This role will involve providing care in patient's homes and as well as in different facilities in the area.

Responsibilities: 
<ul>
<li>Performs a comprehensive assessment and coordinates the plan of care with all disciplines associated with patient's needs, including family and primary caregivers.</li>
<li>Observe, evaluate and report on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever change occurs.</li>
<li>Provides skilled nursing care including administration of medications and treatments as prescribed in physician's plan of care for patients in their home setting.</li>
<li>Participates in weekly Interdisciplinary Team Meetings, providing updates on patient's physical, emotional, psychosocial, and spiritual status.</li>
<li>Maintains and processes all documentation in compliance with agency standards</li>
<li>Will utilize computer based documentation system and submit all required clinical documentation, time records and other required information in required time frame.</li>
<li>May act as a liaison with institutions to promote continuity of care as the patient moves from level of care to another.</li>
<li>May participate in community activities as a representative of the agency.</li>
<li>May perform other duties as assigned.</li>
</ul>

Qualifications: 
<ul>
<li> Master’s degree, post-masters certificate, or doctoral degree in nursing from a university accredited by the Commission on Collegiate Nursing Education (CCNE) or by the National League for Nursing Accrediting Commission (NLNAC)</li>
<li> Current license to practice nursing in the state where the hospice program operates</li>
<li> Current National Board Certification from ANCC or AANP as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP)</li>
<li> Experience in hospice or palliative care, clinical oncology, geriatrics, intensive care or community and family health preferred.</li>
<li> Palliative care certification (NBCHPN) or hospice certification (ACHPN®) preferred.</li>
<li> Full participation in and completion of initial credentialing must be accomplished; additionally, subsequent re-credentialing will be accomplished every two years</li>
