Position Scope: The Assistant Health and Wellness Director is responsible for providing nursing support as directed by the Health and Wellness Director. RESPONSIBILITIES:

<li>Assists in re-ordering of medication.</li>
<li>Supervises and maintains cleanliness of medication room.</li>
<li>Ensures that all new medication orders are faxed to the pharmacy in an effort to obtain medication timely and accurately.</li>
<li>Informs families when mail order medicines need to be reordered.</li>
<li>Communicates with physician offices as needed to obtain phone orders or change orders for a resident.</li>
<li>Follows Physician Order Protocol to ensure proper processing of orders.</li>
<li>Documents resident change of condition / status per policy.</li>
<li>Assists in completing chart audits and thinning per policy.</li>
<li>Assist with the completion of required assessments and service plans.</li>
<li>Assist with the development and maintenance of resident service plan / care plan.</li>
<li>Assists with documenting incident reports correctly and timely.</li>
<li>Updates DNR'S as needed</li>
<li>Assists Health and Wellness Director in creating and maintaining of resident charts/care files</li>
<li>Visits residents in hospital/rehab. as assigned.</li>
<li>Participates in annual flu clinics, TB testing of associates/residents.</li>
<li>Participates in community Wellness Programs as assigned.</li>
<li>Participates in MOD weekend and holiday coverage, as assigned.</li>
<li>Member of the backup sales team</li>
<li>Participates in on-call schedule as assigned.</li>
<li>Obtains and records residents’ vital signs (temperature, blood pressure, pulse, and respiration), weight, as directed.</li>
<li>Observes resident to detect response to specified types of medications and prepares report or notifies designated personnel of unexpected reactions.</li>
<li>Counsels' residents in identifying and resolving social or other problems.</li>
<li>Schedules doctor’s appointments based on the needs of the resident.</li>
<li>Assures call lights are answered in a timely manner in an effort to attend to the residents needs as soon as possible after the call light is signaled.</li>
<li>Attends training and conferences as required to stay current with changes in the nursing profession, as well as to maintain appropriate CEU’s for license, with prior approval of supervisor. Assures all training required by the state regulations is completed and documented each year.</li>
<li>Assures all caregiver duties are performed as assigned and documented appropriately.</li>
<li>Assists in the oversight of appropriate training for all nursing staff to assure requirements for state regulations are met, as assigned by supervisor.</li>
<li>Addresses associate, family and resident concerns to promote satisfaction and good communication.</li>
<li>Is fully knowledgeable of the fire evacuation and disaster policy of the community and participates as assigned in fire and disaster drills.</li>
<li>Maintains confidentiality of all pertinent personal or health information concerning residents and staff.</li>
<li>Demonstrates an understanding of compliance and ethics program policies and procedures.</li>
<li>Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.</li>
<li>Assists with recruiting and interviewing candidates.</li>
<li>Attends training courses at community and maintains up to date monthly, quarterly, annual training through designated Learning Management System (LMS).</li>
<li>Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.</li>

QUALIFICATIONS:

DUTIES
JOB KNOWLEDGE AND ROLE RESPONSIBILITIES:
1.Demonstrates knowledge, skills, and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease and wound care, if applicable.
2.Under the direction of the DON, ensures that a sufficient number of qualified supervisory and supportive nursing personnel are assigned for each unit/shift to meet the resident needs.
3.Working closely with the DON, makes rounds upon entering the building each day. Checks each nursing unit for staffing, census, unstable residents, accidents and incidents, etc. Implements plans to correct grooming concerns, if necessary.
4.Reviews all Accidents and Incidents (A/I) daily and develops an appropriate plan to prevent future accidents and incidents. Corrects periodic reviews of documentation for inconsistencies on each unit.
5.Participates in all screening processes prior to admission. Assesses items needed by the facility to meet resident needs.
6.Conducts periodic review to ensure all documentation is informative and descriptive of nursing care and of the resident's response to that care.
7.Conducts periodic review to ensure that the nursing requirements of each resident admitted to the facility are reviewed and that the physician is consulted in planning resident care.
8.Audits specific resident care issues on a daily basis such as skin, ambulation, etc. Devises a Monthly Planning Calendar as a minimum guideline for auditing care issues.
9.Introduces self to new admissions within 48 hours. Checks for documentation initiation (i.e. physician orders, initial assignments, nursing care plan, etc).
QUALIFICATIONS
SUMMARY OF QUALIFICATIONS:
1. Maintains a current, valid license as a registered nurse in good standing.
2.Two year degree in nursing required BSN preferred.
3.Possesses a minimum one (1) year of experience in nursing service administration. Additional education or experience in such areas as rehabilitative or geriatric nursing is preferred.
4.Possesses strong knowledge of state, federal and local regulations as they pertain to long term care.
5.Possesses good communication skills and is patient and self-disciplined.
6.Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.

POSITION SUMMARY
The primary responsibility of your job position is to assist with planning, organizing, developing, and directing overall operation of our Nursing Service Department following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. KEY RESPONSIBILITIES:
