<li>The Residential Psychiatric Nurse will perform with a demonstrated ability and sensitivity providing services to highly vulnerable individuals who have history of homelessness, substance affected, and/or have serious and persistent mental illness.</li>
<li>The Residential Psychiatric Nurse will work with culturally and lifestyle diverse populations.</li>
<li>Refer individuals to immediate/urgent care as well as establish community connections with primary care, mental health services and substance use disorder care.</li>
<li>Provide preliminary health diagnosis, care, and treatment plans to clients.</li>
<li>Provide crisis intervention assistance when needed.</li>
<li>Document each client interaction, health data and notes in the AWARDS system.</li>
<li>Coordinate and follow-up on internal and external doctors’ appointments, hospital and rehabilitation admissions of clients including healthcare assessments as needed</li>
<li>Collaborate with the Residential Manager, Buildings and IT Manager and other interdisciplinary staff to prioritize placement meetings for Encore 49.</li>
<li>Establish and maintain community health resources with direct linkages to care.</li>
<li>Participate in internal and external health care management meetings with Senior Director, Program Director, Case Managers, and other care coordinators when needed.</li>
<li>Administer medication as prescribed when needed, including but not limited to ointments, injections, ophthalmic, etc.</li>
<li>Manage and report quarterly on any an assessment and treatment outcomes.</li>
<li>Maintain a safe and confidential working environment that meets federal, state, funding source and Encore Community Services requirements.</li>
<li>Actively participate in the weekly Case Review Meeting along with the monthly Psych meetings with the Psychiatrist.</li>
<li>Create, update, and add addendums to current medical Policy and Procedures, ensuring adherence to state, federal, and Encore 49 requirements and regulations.</li>
<li>Clearly and accurately document all incidents and report in a timely manner</li>
<li>Ensure timely reporting, documentation and record-keeping and hard copy files in accordance as mandated by funding sources or agency administrator, in an orderly and up-to-date manner;</li>
<li>Actively participate in the interdisciplinary team approach at Encore 49, including communicating with the Senior Director, Residential Manager and Buildings and IT Manager.</li>
