Marion County Adult Behavioral Health is seeking an experienced behavioral health RN to provide direct nursing care to adults diagnosed with serious mental illnesses, some of whom have co-occurring substance use disorders. The incumbent performs all of the following within scope of practice and using a person-centered, strengths-based, recovery-oriented, trauma-informed, culturally-sensitive, and ethically-guided approach: </li>
<li>Provides direct nursing care including the following: nursing assessments, taking vitals and weighing patients, administering injections. Provides emotional support, compassion, and de-escalation (when necessary) to patients/guardians/relatives; escorts persons served to psychiatric offices, injection spaces, and restrooms. </li>
<li>Triages and processes incoming requests for psychiatric medication interventions; provides case management.</li>
<li>Prepares, organizes, and manages information/data/records pertinent to psychiatric prescribing and nursing services (psychiatric medication intervention records, injection no-show lists, incident reports, etc.). Prepares schedules to ensure adequate prescriber/RN coverage; takes lead on coordinating case transfers; monitors caseload and patient flow and recommends changes when necessary. For psychiatric team meetings prepares and keeps records related to meeting business (such as the Morbidity and Mortality review records). </li>
<li>Prepares training materials relevant to the following areas: Nursing services; understanding and recognizing mental illness and symptomatology; psychotropic medications, medication management, dispensing, drug interactions, and side effects; medication documentation (including MAR documentation); incident reporting, etc. Provides in-person and technology-based training to nursing, non-nursing, persons served, guardians, and staff in the following settings: clinic and licensed adult foster homes and licensed residential facilities.</li>
<li>Provides training to newly hired nurses; provides guidance and consultation to nurses on complex cases; provides input to the manager/supervisor concerning performance strengths and areas of improvement among nursing staff. </li>
<li>Prepares and writes service records that are clear, legible, accurate, and professional in nature and in accordance with federal regulations, state statutes and rules, and agency policies. </li>
<li>Identifies problems associated with quality of behavioral health services, investigates causes of quality gaps, and develops solutions to timely correct issues; prepares written workflows and recommends improved workflows; monitors implementation activities related to quality improvement. </li>
<li>Liaises between prescribers, counselors, pharmacies, residential facilities, guardians/relatives, and others involved in the patient's care; liaises with hospitals, clinics, laboratories, and primary care providers.</li>
<li>Monitors medical equipment inventory and ensures that inventory records are accurate and complete. Identifies medical and nursing needs of patients and reports these to the agency's Nurse program manager. Assists in coordinating referrals to community providers. </li>
<li>Other job duties as assigned by your supervisor.</li>

<li>Provides individual, family and group therapy services. Completes intake mental health assessments, including screening for alcohol and drug issues. Completes and updates an individual service plan. Obtains and maintains competence in all screening and assessment tools required by the county. Provides case management and outreach services to clients. Assists clients and families with developing skills to access community supports/resources and to advocate for themselves. Assists with making referrals for services.</li>
<li>Provides level of care determinations and eligibility reviews for youth being referred to the Wraparound Program. Presents cases at the System of Care Review Committee, which include eligibility recommendations.</li>
<li>Participates in individual service plan development in collaboration with the treatment team and other community providers. Ensures the collaborative development, periodic review and revision of an individual service plan. Provides service coordination to assist families in accessing formal and informal supports and services; facilitating communication between the family, natural supports, community resources, and involved child-serving providers and agencies; participates in team meetings at which strengths and needs are identified and safety planning occurs; and provides for continuity of care by creating linkages to, and managing transitions between levels of care, and transitions of older youth to the adult service system.</li>
<li>Completes required documentation and documents services to meet contract, administrative rule, and billing requirements and maintains acceptable level of productivity for these requirements. Expectation is that employee will strive for 60% of work hours on average to be billable hours with a minimum of 50% billable hours on average each month.</li>

GENERAL STATEMENT OF DUTIES Mental Health Therapist 2: Provide assessments and treatment of individuals and families, including groups; provides crisis assessment and intervention services and related duties as required. Primary responsibility of this class is case management and counseling, without responsibility for developing and implementing specific programs in the community.
SUPERVISION RECEIVED Mental Health Therapist 2 and 1: Works under the supervision of a supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness.

Typical Duties - Duties include, but are not limited to the following
Enhanced Care Services

<li>Review referrals with OHA/DHS (Oregon Health Authority/Oregon Department of Human Services), and other stakeholders. Coordinate on-site screenings at various settings, including Oregon State Hospital (OSH), Acute Psychiatric Units, and other APD- and MH-licensed treatment programs. Consult with Supervisor on final decisions.</li>
<li>Coordinate admissions with Providence Benedictine Nursing Center (PBNC) staff and referring entity.</li>
<li>Determine treatment needs, diagnose, and develop: Comprehensive Mental Health Assessments, Individualized Service Plans, Behavioral Support Plans, and Crisis Plans, with the goal of transitioning individuals to lower level of care. Update as necessary, with annual required according to Oregon Administrative Rules (OARs).</li>
<li>Meet with individuals on a weekly basis to provide therapy; may also provide case management, group therapy, and/or skills training as determined by the Individualized Service Plan. Provide crisis stabilization when necessary.</li>
<li>Oversee and coordinate daily functioning and activity programs with residents on the unit including outings with Qualified Mental Health Associates (QMHA) and PBNC staff. Facilitate daily huddles with QMHAs and Nursing Staff to identify priorities.</li>
<li>Coordinate and facilitate weekly IDT (Inter-Disciplinary Team) meetings to review treatment progress of residents, as required by OARs. All residents have to be reviewed monthly. Complete Transition Tool quarterly for each resident. Record notes for dissemination for all participants.</li>
<li>Facilitate Learning Circles (MH training for PBNC staff) with Supervisor, QMHAs, and/or Older Adult Behavioral Health Coordinator. Frequency will be determined by needs of unit, at minimum quarterly. Attend Focus Meetings (facilitated by PBNC) as necessary.</li>
<li>Determine, with consultation with Supervisor and Prescriber (Psychiatric Mental Health Nurse Practitioner (PMHNP) or Psychiatrist), appropriateness of continued enrollment in ECS.</li>
<li>Coordinate discharge planning with NorthWest Seniors and Disability Services (NWSDS) Case Manager (CM) and Providence Benedictine Nursing Care staff. Identify MH needs to be addressed at future placements; relay MH, Neurocognitive, and Physical Health needs to NWSDS CM. (Physical Health and Neurocognitive needs will be identified by PBNC and other appropriate entities.)</li>
<li>Assist in screening residents for discharge. Explore placement options identified by NWSDS CM and review with treatment team, resident, and legal representative.</li>
<li>For all of the above, complete clinical documentation professionally and accurately in a timely manner as directed by Marion County HHS policy. Submit paperwork to OHA/HSD or other entities as required. Maintain accurate and comprehensive MH charts. If necessary, transports charts following HIPAA regulations to support staff at Marion County ABH.</li>
<li>Submit Monthly Census to OHA/DHS.</li>
</ul>
Enhanced Care Outreach Services
<ul>
