Occupational Therapist 
POSITION SUMMARY: Plans, organizes, and implements Occupational Therapy services for PACE participants as a member of the PACE interdisciplinary team. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching of participant, caregivers or other appropriate representatives/family to maintain participant support in the community. 
ESSESNTIAL DUTIES AND RESPONSIBILITIES: 
<ul>
<li>Conduct initial and periodic in-person assessments to evaluate participants’ functional abilities, home environment, and rehabilitation needs.</li>
<li>Recommend and coordinate adaptive equipment, home modifications, and therapy services to promote safety and independence.</li>
<li>Develop, implement, and revise individualized treatment plans based on participant progress and IDT collaboration.</li>
<li>Deliver OT interventions across settings including the PACE center, clinic, participants’ homes, and contracted facilities.</li>
<li>Educate participants and caregivers on equipment use, home safety strategies, and ADL programs.</li>
<li>Maintain timely, accurate documentation of assessments, treatment, and participant progress per regulatory and organizational standards.</li>
<li>Actively participate in daily IDT meetings, contributing to comprehensive care planning and 24-hour care coordination.</li>
<li>Supervise Certified Occupational Therapy Assistants (COTAs), including review of care plans, co-signing documentation, and providing ongoing mentorship.</li>
<li>Train staff in safe OT practices and equipment use to enhance participant care.</li>
<li>Support quality improvement, utilization review, and compliance initiatives.</li>
<li>Perform other IDT duties as needed within scope of practice and competencies.</li>
<li>Maintain current licensure, certifications, and participation in ongoing professional development.</li>
</ul>

EXPERIENCE AND EDUCATION: 
<ul>
<li>Advanced degree in Occupational Therapy from an accredited school of Occupational Therapy</li>
<li>Current license to practice Occupational Therapy in the state</li>
<li>Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.</li>
<li>1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring (If applicable for the role).  </li>
<li>Competency position specific competencies for the Occupational Therapist will be met prior to assuming participant care. </li>

JOB SUMMARY
Plans, organizes, and implements Physical Therapy services for PACE participants. ESSENTIAL DUTIES AND RESPONSIBILITIES:

<li>Performs in-person assessments for participants at initial enrollment and as-needed ongoing as determined by the IDT. </li>
<li>Assesses the need for adaptive durable medical equipment, specialized shoes and prosthesis. Ability to assess participant’s home for safety and mobility. Recommend home modifications and equipment. </li>
<li>Plans for physical therapy treatment programs according to recognized standards and policies. </li>
<li>Administers physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies. Confer with interdisciplinary team on participant care issues. </li>
<li>Documents evaluation, assessment, plan and treatment rendered accurately and in timely according to policy. </li>
<li>Measures effectiveness of treatment, document and revise plan accordingly. </li>
<li>Educates and instructs participants/family in procedures, home programs and document training and understanding by participant/family.</li>
<li>Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL’s). </li>
<li>Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care. </li>
<li>Assists participant to function at most independent level possible. </li>

<li>Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community. </li>
<li>Assists the interdisciplinary team members in understanding the significant rehabilitation needs of the participant and families </li>
<li>Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.</li>
<li>Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborates with team members in care planning decisions and coordination for 24-hour care delivery.</li>
<li>Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed.</li>
<li>Actively participates in quality improvement projects and meetings. </li>

INTRODUCTION
Under the supervision of the Nurse Manager and Health Center Director, the Medical Assistant provides basic patient care in a community health center setting. Working with health care providers and other health center employees, the Medical Assistant is an integral part of the patient care team. The Medical Assistant is directly responsible for, but not limited to, initiating medical history, vital signs, height, weight, preparing patients for examination, reporting special problems or complaints to providers, assist in making appropriate health center/referral appointments for patients, and assist with follow up of missed appointments.

MAJOR DUTIES/ESSENTIAL FUNCTIONS 
Essential and other important responsibilities and duties may include, but are not limited to the following:

<li>Maintains patient and Electronic Health Record (EHR) confidentiality.</li>
<li>Prepares patient and EHR for provider visit, reports special problems or complaints to the provider, and assists during exam as needed.</li>
<li>Accurately performs and documents basic vital signs, height, weight, chief complaint, and initiates appropriate medical and social history for adult and pediatric patients.</li>
<li>Performs routine lab and patient intake procedures (i.e. standing orders, urine dipsticks, phlebotomy, finger/heel sticks, injections/immunizations, and EKG). </li>
<li>Maintains proper cleanliness and organization of office and medical areas including proactively restocking supplies and Family Planning resource center as needed.</li>
<li>Executes Pre-Visit Planning for Primary Provider and Patient -Centered Medical Home (PCMH) team. </li>
<li>Regularly provides basic chronic disease health education to patients as necessary.</li>
<li>Assists with processing appropriate referrals and appointments for patients, obtaining authorizations as needed, and follows up to determine if appointments were kept as scheduled.</li>
<li>Communicates and assists in effective discharge of the patient.</li>
<li>Maintains all logs and required checks (i.e. refrigerator temperatures, eye wash stations, oxygen, autoclave fluid change and spore testing, etc.)</li>

AdvisaCare is seeking Top of Line Licensed Practical Nurse/ Registered Nurse to service our clients for Private Duty In-Home Care in the Farmington Hills, MI area. If you believe each patient is as unique as the care they need - strive to offer the right care customized to each individual all while honoring his or her preferences, choices, abilities and specific needs - YOU'RE THE ONE we're looking for!!
-MUST be willing to travel between both parents homes (Farmington Hills, MI and Milford, MI)
-MUST be available on weekends
-MUST be willing to work 12 hour shifts
-Part time available
Requirements
<ul>
<li>Current MI Nursing License</li>
<li>High School Diploma or GED</li>
<li>Reliable transportation &amp; valid driver’s license</li>
<li>Ability to work independently and in a team</li>
<li>Able to pass drug screen (Including THC) and background check</li>

LakeHouse Senior Living is hiring CNAs for our community Walker Place Senior Living.
The CNA's role includes providing hands-on care, physical and emotional support to each resident while maintaining a safe and comfortable home-like environment.
Responsibilities:
<ul>
<li>Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift.</li>
<li>Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities.</li>
<li>Responds to security system and resident call bells promptly.</li>
<li>Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs.</li>
<li>Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms.</li>
<li>Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.</li>
<li>Maintains cleanliness of resident’s room and work areas.</li>
<li>Practices good standard care precautions of cleanliness, hygiene, and health.</li>
<li>Helps residents maintain independence, promotes dignity and physical safety of each resident.</li>
<li>Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed.</li>
<li>Engages residents in life skills and other life enrichment activities.</li>
<li>Greets and assists all internal and external customers, guests, family members, residents, vendors and team members.</li>
<li>Does resident laundry as assigned and needed.</li>
<li>Other duties as assigned by the Supervisor.</li>
</ul>
Qualifications:
<ul>
<li>Certified Nurse’s Aid certification or Home Health Aide.</li>
<li>High School diploma/GED</li>
<li>Must be 18 years of age.</li>
<li>Previous experience working with seniors preferred.</li>
</ul>
