Responsibilities: 
<ul>
<li>Drive to Senior Living Communities to provide on-site primary care to residents </li>
<li>Facilitate the patient's plan of care: chronic care management, advanced treatment, and end-of-life planning </li>
<li>Coordinate with the interdisciplinary team, reconcile medications, and order labs </li>
<li>Communicate clearly and regularly with Physician, office support staff, community staff, and patients' families </li>
</ul>
Qualifications: 
<ul>
<li>Licensed as a Certified Nurse Practitioner / CNP in the State of Minnesota </li>
<li>Experience in Geriatrics and/or Primary Care </li>
<li>Valid driver’s license and current auto insurance </li>
<li>Passion for serving the senior population </li>
<li>Strong interpersonal communication skills </li>
</ul>

The Veteran files the claim before they see you. All of their medical records are uploaded into a computer portal and you will be able to review these during the appointment. You will conduct a Maryland CNC Test and Puretone Audiometric test. The Audiologist will complete a Disability Benefits Questionnaire (DBQ) in the computer portal. This forms the medical evidence that allows the VA to decide the veteran’s benefits.
Audiologist will NOT prescribe any medical treatments or devices.
This position will review and assess 3-6 Veterans per day on average.
Schedule: 3 consistent days/week | M-F: 8am-5pm
This is an IN-PERSON position that offers: 1 hour to 1 hour &amp; 30 minutes per patient. No evenings, no on-call, no weekends, no hearing aid sales
