Summary:
Under the supervision of the Treatment Director, Lead Nurse, &amp; Assistant Medical Director, this individual will administer prescribed medication and nursing care utilizing the nursing processes according to protocols established by the Assistant Medical Director and Treatment Director. Implement plans of care and evaluate the response to care given. Refer patients with medical needs not provided by the facility to appropriate levels of care.
Essential Duties and Responsibilities:

<li>Assess, plan, implement, monitor, and evaluate medical and nursing care of all patients.</li>
<li>Explain procedures and treatments to patients to gain cooperation understanding and to alleviate apprehension.</li>

<li>Maintain PMP access and manage delegates;</li>
<li>Nursing education and instruction as needed;</li>
<li>Collaborate with community physicians, CSAT, social workers internal and external to CADT, pharmacists/toxicologists, and other health professionals for patient care;</li>
<li>Patient and family education;</li>
<li>Provide biopsychosocial evaluations for adults at substance abuse clinic; and</li>

Qualifications:
<ul>
<li>A current, unrestricted license to practice as an advanced practice nurse in the state of Minnesota;</li>
<li>A minimum of five (5) years’ experience in direct patient care;</li>
<li>At least one (1) year of experience providing Medication Assisted Treatment or prior work in a substance use clinic or hospital setting;</li>
<li>Holds a DATA waiver for providing Suboxone or is willing to seek immediate certification;</li>
<li>Knowledge of managed healthcare systems, medical quality assurance, quality improvement, and risk management;</li>
<li>Strategic and innovative thinker with the ability to communicate a vision and drive results;</li>
<li>Ability to problem-solve and work collaboratively across disciplines;</li>

Summary:
This individual performs medical administration for patients of the Center of Alcohol and Drug Treatment. Oversee medical care of patients and promote optimal health and refer patients needing a higher level of care. Manage difficult patients and oversee LPNs and other support staff as needed.
Essential Duties and Responsibilities:

<li>Obtain and accurately document a relevant health history from patients; perform a Comprehensive physical and mental health assessment.</li>
<li>Identify medical and mental health problems and develop/implement a plan of care to minimize complications and promote optimal health. Immediately refer patients needing a higher level of care to appropriate facilities.</li>
<li>Counsel patients and use the therapeutic relationship to promote health.</li>
<li>Teach/coach patients and family members to impart knowledge and associated skills.</li>
<li>Collaborate as a member of the interdisciplinary team.</li>
<li>Appropriately implement protocols and order. Review licensed practitioner orders, monitor documentation, and ensure enforcement of rules and regulations, including staffing requirements.</li>

<li>Provide and direct healthcare of patients, including supervision of the nursing care.</li>

In this role, you will play a key part in supporting patients on their journey to recovery by providing direction, guidance, and therapeutic support within our Clearpath program. Clearpath is a non-profit Opioid Treatment Program (OTP) using Medications for Opioid Use Disorders (MOUDs) alongside counseling services to treat patient Opioid Use Disorders.&nbsp; Summary The Center is looking for compassionate LADC/ADC-T to help provide direction and support to our patients. You will actively help patients build coping mechanisms to guide them through stressful situations. Your duties will also include listening to patients, asking necessary questions, and developing effective strategies to improve their lives. As a Counselor within Clearpath you will work with patients specifically with an opioid substance abuse. You'll manage of a case load up to 60 patients and meet with them once a month at the minimum. The CORE FUNCTIONS of this position are as follows: screening, intake, orientation, assessment, treatment planning, counseling (group, individual and family), case management, crisis intervention, client education, referral, reports, record keeping, and consultation with other professionals Programs will include SUD and Mental health services Responsibilities:
