Responsibilities:
Every day is different for a Home Health Nurse. Our Home Health Nurses care for ill, injured or convalescing patients or persons with disabilities in private homes. You will be responsible for providing quality healthcare to each patient in adherence with all applicable laws, regulations, and policies.

Job Description for Acknowledgement:
1. Education
a. Graduate of an accredited school of practical or vocational nursing
b. Current licensure with the State Board of Nursing in the state in which the LPN/LVN practices
2. Experience
a. One year’s experience as an LPN/LVN in a supervised setting within the last three (3) years
b. Successful completion of the skills assessment specific to the area of practice
3. Position Description
The practice of nursing as a licensed practical nurse/licensed vocational nurse is defined a performing tasks and responsibilities within the framework of case-finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed physician, dentist or other licensed health care provider legally authorized per State/Federal regulatory entity.
4. Duties & Responsibilities
a. Respects client’s rights and maintains confidentiality
b. Receives and provides complete reports on assigned clients
c. Provides Nursing Interventions and treatments per the Plan of Care in accordance with agency policies
d. Participates in client care planning and coordination of services
e. Notifies supervisor of changes in client’s condition
f. Updates and documents client care per the Plan of Care
g. Understands and uses aseptic techniques
h. Prepares equipment and materials for treatment
i. Complies with the guidelines and ensures that both the company’s and the client’s equipment and supplies are safeguarded
j. Records accurately all pertinent findings in the client’s clinical record
k. Prepares clinical and progress notes
l. Attends in-service programs and informs the office of newly acquired skills
m. Assists the client in learning appropriate self-care techniques
n. Receives verbal doctor’s orders
o. Performs other duties as assigned by the immediate supervisor
5. Routine Functions
a. Performs routine nursing duties including but not limited to:
1. Assistance with ADL’s and IADL’s
2. Medication administration
3. Vital sign, intake and output, and other assessments
4. Client and caregiver health instruction
5. Collecting specimens
6. Changing dressings
7. Client exercises
b. Notifies the Registered Nurse or Clinical Director of any changes in client’s condition
c. Performs the following where training or demonstrated skills and the state nurse practice act permits:
1. Administering and recording medications (other than IV medications)
2. Inserting and irrigating tubes and Foley catheters
3. Administering oxygen by mask or cannula
4. Ventilator, tracheostomy care and suctioning endotracheally
5. Changing dressing (sterile or non-sterile)
6. Supervision of aides and other health care team members
d. The following tasks are also considered essential functions of the position:
1. Lifting unspecified weights, depending on assignment requirements
2. Bending and stooping to prepare equipment and materials for treating clients or managing the client’s environment
3. Transferring, turning and positioning clients and assisting clients in walking and moving from place to place
6. Supervision
The LPN/LVN is supervised by a Registered Nurse or the Clinical Director
7. Evaluation
The LPN/LVN’s job performance is evaluated by the supervising Registered Nurse or the Clinical Director using input from client, office staff, and personal observations
