Inspiration is looking for a Hospice Chaplain in the San Antonio area. The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Inspiration Hospice, either directly or through coordination of care with other spiritual counselors.



Job Summary: To provide spiritual and emotional support when needed to the hospice patient/family and to members of the hospice team. To provide assistance at the time of the patient’s death and support of the family during the bereavement period.


Duties include, but not limited to:

  • Assures spiritual assessment of patients and families/caregivers in the hospice program, and appropriate services are rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provide direct spiritual support and/or counsel to patients/families in keeping with patients/families' beliefs.
  • Work with staff, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
  • Recruits' community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attends/participates in patient care conference and interdisciplinary group meetings by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated.
  • Actively participates in quality assessment, performance improvement teams, and activities.
  • Provides consultation, education, and support to the interdisciplinary group on spiritual care.
  • Provide bereavement follow-up services as assigned.
  • Maintain proper records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Make contact with clergy or appropriate representative of patients/families as indicated.
  • Perform occasional liturgical assignments, e.g., memorial services when indicated.
  • Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  • Develop and maintain a resource group of clergies to whom specific aspects of spiritual care may be delegated.
  • Arrange for on-call availability of spiritual services.
  • Provide educational programs for community clergy, religious, and lay representatives as resources allow.
  • Assists in the supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
  • Accepts and performs other related duties and responsibilities as required.

Requirements:

Education:

  • Graduate of an accredited seminary or school of theology (MDiv or equivalent theological degree)
  • 2 CPE units (Required)
  • Board Certified (Preferred)

Experience:

  • Experience working with death and dying
  • Must demonstrate the ability to chart in a clinical and professional fashion.
  • Must be a self-starter. This position requires the candidate to coordinate care, attend meetings, and handle emergent situations as needed.
  • Ability to demonstrate clear boundaries in stressful and team settings.
  • Ability to work as a member of an interdisciplinary team/group.
  • Comfort in an interfaith setting.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • Comfortable with different types of technology.

Physical Requirements:

  • Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.

  • Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

  • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.

Transportation:

  • Must have a current valid driver’s license, auto liability insurance and reliable transportation.
