Sunbelt Staffing is partnering with a reputable national home health agency to offer a full-time, 13-week contract position in O'Fallon, IL. This opportunity includes the potential for contract extension based on performance, providing stability and growth in the rewarding field of home health care.

Job Details:

  • Location: O'Fallon, IL,and surrounding areas
  • Hiring Company: Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
  • Position Type: Contract, 13-week term with possibility of extension upon successful completion
  • Schedule: Monday-Friday, 8:00 AM - 5:00 PM with occasional weekends and on-call
  • Compensation: Competitive salary, mileage reimbursement, medical, dental, vision, and 401 (k) benefits after 90 days
  • Productivity: 25-32 points per week, up to 35 patients
  • Mileage: .48c visit to visit, 150-250 miles per week expected travel

Job Responsibilities:

  • Conduct home health visits to patients within the territory and its surrounding areas.
  • Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being.
  • Develop and execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.
  • Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures.
  • Maintain accurate and timely documentation of all patient interactions, assessments, interventions, and outcomes utilizing Oasis and Homecare Homebase software platforms.
  • Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery.
  • Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey.

Job Requirements:

  • Active IllinoisRegistered Nurse (RN) License.
  • Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
  • Minimum of 1 year of experience in home health in a field setting
  • Minimum of 1 year of experience with Oasis and Homecare Homebase software, proficiency required.
  • Excellent clinical assessment and critical thinking skills.
  • Strong communication and interpersonal abilities.
  • Reliable transportation and willingness to travel within the assigned territory.

Please Note: This is a contract position with limited training. Prior home health experience, along with proficiency in OASIS documentation and Homecare Homebase (HCHB), is required for consideration.
