Job DescriptionHealth / Human Service Case Manager ILocation: Newark, DelawareSchedule: Monday – Friday (8am to 4:30pm) with a one-hour lunch. Rate of Pay: $20 an hour
Job Responsibilities:
- Responsible for case management by assisting clients to meet personal, social, health and economic needs.
- Applying person-centered planning approach to support and engage with Delawareans needing services to age in place
- Makes referrals and assists clients in applying for LTSS Medicaid.
- Maintains a caseload, determines initial and ongoing eligibility for services.
- Conduct home visits to client’s location.
- Complete activities by the due date.
- Respond to routine calls/emails within 2 business days of contact.
- Utilize companies database, Outlook Calendar, Tracking Assessing Planning (TAP), ASSIST Worker Web (AWW), Vital Statistics, and Delaware Medicaid Enterprise System, (DMES), for casework activity.
- Check New Referral saved search daily to retrieve referral.
- Contact participant to complete an application over the phone and in person.
- Monitor application process from start to decision including gathering documents as requested by DMMA for processing.
- Other duties assigned by the supervisor
Knowledge, Skills and Abilities:
- Knowledge of principles, practices, methods and techniques of social work.
- Knowledge of Federal/State eligibility and assistance requirements
- Knowledge of agency, community resources, and eligibility requirements
- Skill in writing objective narratives to document the application process as well as completing logs, reports, and records as needed.
- Skill in interviewing applicants and analyzing, assessing, and identifying needs.
- Skill in establishing effective working relationships with co-professionals and clients.
Job Requirements: Have education, training or experience demonstrating competence in the following areas.
- Must have 6 months or more of Case Management experience.
- Must have a valid drivers license and ability to drive a State vehicle.
- Six months experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
- Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
- Must have narrative writing skills.
- Must pass drug test.
