Our organization is in search of a dedicated and empathetic Home Health Physical Therapist to become a part of our Healthcare & Medical Services team. In this position, you will provide personalized rehabilitative care directly in the homes of our patients.
Key Responsibilities
- Assess adult and elderly patients to evaluate their physical therapy needs.
- Design and implement tailored care plans based on individual patient requirements.
- Deliver evidence-based therapeutic interventions aimed at enhancing mobility, strength, pain management, and overall functional independence.
- Work collaboratively with nurses, occupational and speech therapists, physicians, and caregivers to ensure comprehensive and coordinated care.
- Conduct thorough physical therapy evaluations and establish measurable functional goals.
- Educate patients and their families on home exercise programs and safety practices.
- Monitor patient progress and modify treatment plans as necessary.
- Maintain accurate and timely documentation through electronic medical records.
- Communicate any changes in patient condition to the interdisciplinary team.
- Participate in care conferences to facilitate optimal patient outcomes.
- Promote fall prevention, safe transfers, and the proper use of assistive devices to help patients remain safely at home.
Qualifications and Requirements
- Licensed Physical Therapist with relevant experience.
- Strong clinical skills and the ability to work independently.
