MAJOR RESPONSIBILITIES:

The Transportation Coordinator (CHI) will be responsible to complete the services intake form for medical transportation, housing and emergency financial assistance. The Transportation Coordinator will work with the transportation coordinator to coordinate transportation services. The Transportation Coordinator will be responsible for the Housing and Emergency Financial Assistance components. Responsibilities is to provide case management and support services. Engaging and re-engaging clients, client assessments, monitoring client's goals, monitoring outcomes, developing an individualized housing plan, monitoring viral suppression. Making referrals as needed. Assist clients with emergency financial assistance for all client's urgent needs. This position is needed to obtain program intake data. The Transportation Coordinator will be responsible for all services provide transitional, short-term, or emergency housing assistance to enable a client or family to gain or maintain outpatient/ambulatory health services and treatment, including temporary assistance necessary to prevent homelessness and to gain or maintain access to medical care. Activities will include the development of an individualized housing plan, updated annually, to guide the client's linkage to permanent housing. Transportation Coordinator will demonstrate courteous and professional communications at all times both on the phone and in face-to-face situations. Job description and duties subject to change based on grant funding and agency need. Must maintain strict consumer confidentiality.

DETAILED RESPONSIBILITES:

  • Respond to incoming calls from potential clients seeking support services;
  • Conduct initial screenings/intake to determine client's edibility for program services;
  • Complete all necessary intake paperwork, collect all client identifying documentation and provide an initial program orientation;
  • Refer clients in need of transportation-to-Transportation Coordinator;
  • Collaborate with transportation coordinator to provide support services to in-house clients;
  • Conduct a needs assessment with client to identify urgent needs/social determinants of health
  • Assist clients in determining eligibility and applying for in-house emergency financial assistance program and/or make appropriate referrals to outside resources/utilize the Unite Us referral platform;
  • Assist clients in determining eligibility and applying for housing and types of housing which meets their needs;
  • When necessary, facilitate and conduct housing search with clients per individual needs, including assisting with application completion;
  • Provide case management to clients that support forward progress on goals aligned with their individualized housing plan;
  • Monitor client's viral suppression;
  • Track clients progress towards goals in detailed case files, with charts, progress notes, and outcome evaluation;
  • Work with client to become document ready by accumulating the documents required for facilitating placement;
  • Outreach, collaborate, build, and maintain relationships with landlords, housing programs and other community agencies.
  • Log, track and monitor all housing referrals till final disposition;
  • Accurately record and maintain documentation, files and data collection as required by funding source;
  • Complete internal reports as assigned in timely manner on a monthly basis;
  • Participate in required meetings as determined by funding source and supervisor.
  • Track client activities and ensure timely and accurate collection of program data as outlined by management team.
  • Collect and input client data into agency approved database in a timely manner, by the end of the week the service was provided.
  • Participant in EOC's Consumer Advisory Board Meetings (CAB);
  • Participate in in-service trainings and on-site workshops as required for professional growth and development. Attend external trainings and workshops at direction of management;
  • Participate in minimum of 1-hour biweekly supervision and 1- hour of group supervision per month;
  • Attend programs, departmental agency meetings, and perform other duties as assigned by the supervisor and/or required by agency policies and procedures;
  • Ensure the confidentiality and safety of case files and client information and obtain consent for any needed referrals;
  • Provide data entry support when needed;
  • Maintains strict confidentiality of all client related information;
  • Other responsibilities as directed by supervisor

QUALIFICATIONS:

  • Case management experience preferred;
  • Dedicated to the details and masterful at multi-tasking. Ability to organize and prioritize tasks to meet deadlines.
  • Knowledge of community resources and referrals for housing programs, entitlement programs and additional appropriate client-centered referrals;
  • Ability to work as part of a team with co-workers and with other colleagues as necessary;
  • Bilingual (English/ Spanish) a plus
  • Excellent verbal and written communication skills, including strong interpersonal and organizational skills;
  • Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration statues, and physical abilities in a multicultural environment;
  • Familiar with Word, Excel, and Power Point and experience with data management systems
  • Ability to follow directions and communicate well (orally and in writing);


  • Required Valid New York Driver's License and Insured Vehicle

EDUCATION/TRAINING/EXPERIENCE:

Associates Degree in Human Services or related field and/or 5 years' experience accepted in lieu of degree. Strong written, oral communication, public speaking a plus.
