The Director, Population Health plays a critical role in advancing TVHC’s commitment to building healthier communities and promoting health equity through high-value, patient-centered services. This leader will oversee the design, implementation, and continuous improvement of a comprehensive population health program that strengthens outcomes across the organization.
Working in a highly collaborative, matrixed environment, the Director will partner closely with clinical, operational, and business teams to drive measurable improvements in population health, patient experience, care team experience, health equity, and enhance cost effectiveness. Through this work, the Director will help position TVHC for ongoing clinical transformation and long-term success in value-based care.

Compensation: $135,133.60 - $160,133.60 annualized, depending on experience.
TVHC offers compensation ranges that are determined by a thorough market-based analysis and are fully disclosed in accordance with California law. The pay for a selected candidate is determined by a variety of factors to ensure fair and equitable compensation. Responsibilities:
- Assists senior leadership in system-wide transformation to sustainable healthcare delivery model(s) in a Federally Qualified Health Center setting.
- Participates in the development of short-term and long-range goals, objectives and operational plans.
- Oversees the day-to-day operational processes related to care management, performance improvement, population health analytics, related contracted vendors, state and federal programs, and assigned grant programs that impact population health.
- Leads the population health operations, chronic condition management, and complex care management to meet our patients where they are at on their terms in their care journey.
- Leverages technology resources available and partners with IS team to develop dashboards and platforms that support staff and programs.
- Serves as a liaison with payors, MSOs, community-based organizations, and other healthcare providers to develop partnerships and programs in support of our mission and values.
- Works with dyad clinical and operational leaders to improve quality, experience, effectiveness, and efficiency.
- Applies clinical expertise and sound professional judgment, within the RN/LCSW scope of practice, to support care management and the clinical aspects of population health programs.
- Provides day-to-day leadership, direction, and support to care management and population health staff, including coaching, workflow guidance, and oversight of assigned work.
- Other duties as assigned.
Requirements:
- RN or LCSW required.
- BLS certification required.
- Minimum of five (5) years of progressive management experience within a healthcare provider, payer, association, or vendor setting required.
- Project management and performance improvement experience highly preferred.
- Experience implementing and managing information systems in complex environments.
- Experience with various value-based care delivery models and payment arrangements, including working with government programs and products.
- Ability to travel to designated locations as directed by supervisor.
Qualifications:
- Demonstrated ability to assess organizational needs, design and implement programs and evaluate results.
- Ability to influence change on a large scale and successfully promote with teams of healthcare professionals.
- Experience in developing analytics tools and approaches to identify population cohorts, examine patterns, model outcomes, and evaluate results.
- Experience and demonstrated leadership in quality transformation, delivery transformation and/or population health programs.
- Experience with major health information technology platforms, including care management, performance improvement, analytics, health information exchange and electronic medical record.
- Ability to translate vision into a strategic plan and execute on that plan.
- Demonstrated leadership skills, particularly in cultivating a high-performing leadership team, and in developing and maintaining excellent partnerships with providers, professionals, and staff.
- High degree of resilience, persistence, and ability to thrive in an environment of rapid change.
- Experience in working with culturally diverse populations and delivery health equity.
- Knowledge of various healthcare settings associated workflows for provision of care.
- Demonstrated use of Microsoft Office Suite, Power BI, and Tableau
- Exceptional verbal and written communication skills
- Ability to clearly communicate and find reasonable solutions to complex situations as they arise.
- Ability to represent the organization in a positive manner, within the organization as well as in the community.
