Job Description

Job SummaryThe Family Nurse Practitioner (FNP) provides comprehensive healthcare services to patients across the lifespan, including diagnosis, treatment, and management of acute and chronic conditions. The FNP works collaboratively with physicians, healthcare staff, and support personnel to ensure high-quality, patient-centered care. The role involves patient education, health promotion, and preventive care, tailored to the diverse needs of the community served by Fair Hill Community Physicians (FHCP).


Qualifications:\t

Education

  • Master’s Degree in Nursing (MSN) from an accredited program (Required)
  • Doctorate of Nursing Practice (DNP) (Preferred)


Licenses and Certifications

·Certified Family Nurse Practitioner (FNP-BC) (Required)·

Active RN license in Pennsylvania (Required)·

DEA Registration (Preferred)


Experience·

2+ years of experience as a Family Nurse Practitioner (Preferred)·

Experience in primary care, family medicine, or community health settings (Preferred)

Essential Functions

Patient Care

· Conducts comprehensive physical exams and health assessments.

· Develops individualized care plans and prescribes treatment as needed.

· Manages acute and chronic health conditions, including medication management.

· Provides patient counseling on lifestyle changes and preventive care.

· Orders and interprets diagnostic tests, including lab work and imaging.


Clinical Documentation

· Maintains accurate and complete medical records.

· Documents patient encounters in the EHR promptly and accurately.

· Ensures compliance with FHCP policies and healthcare regulations.

Health Promotion & Education

· Educates patients and families on chronic disease management.

· Promotes preventive health practices, including vaccinations and screenings.

· Collaborates with community health initiatives to improve population health.

General Responsibilities:

· Complies with all FHCP and healthcare regulations, including HIPAA.

· Participates in professional development and continuing education.

· Attends staff meetings and contributes to clinical quality improvement.

· Performs other duties as assigned.
