Housing Navigator
Supportive Services-Housing and Homeless Department
Summary
The Housing/Healthcare Navigator assist and advocated on behalf of One Stop Day Shelter participates. They will provide housing counseling, information and referral services. The Navigator plans and coordinates services, identify appropriate housing options, and interacts with landlord and other organizations on behalf of participants.

Essential Functions
- Maintain a caseload of at least 20 participants
- Complete intake and initial assessment of service needs
- Develop and monitor service plans in accordance with regulatory guidelines
- Develop and maintain accurate and current housing portfolio. Provide participants with a listing of potential housing options in alignment with their needs and geographic preference
- Help participants to mitigate issues with credit reports, utility arrears, criminal records, unfavorable landlord references, and other issues which may impact housing eligibility
- Assist participants with apply for benefits such as SNAP and Medicaid
- Assist participants with gathering all needed housing documents such as social security card, birth certificates, picture id etc. Assist individuals with housing applications.
- Assist participants in their housing search, including applying for apartments, negotiating with potential landlords, and accompanying participants to apartment view as needed
- Conduct regular in-person post placement visits, mitigates risks, and help improve the likelihood of housing retention
- Maintain and report complete and accurate documentation of services using the HMIS (Homeless Management Information System) database
- Maintain professional, courteous relationship with all consumers, staff, community members, and professional partnerships
- Assist the Program Supervisor with APR report
- Assist with budget monitoring and planning
- Following all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time cards, requesting edit sheets, etc.)

Required Education and Experience
Bachelor’s degree in the Human Service Field, one-year experience working with homeless population and/or individuals with disabilities is required. Must have valid driver’s license and current vehicle insurance.

The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
