Job Description

LPN/LVN Home Health, Lake Oswego, OR - Local or Travel

Backfill order

Must be flexible to cover all territories under Lake Oswego branch
13 weeks
Monday thru Friday, 8am - 5pm, 40 hours per week.

one position for East side and one for West side
Home health required
HCHB desired
On call required
28-30 units per week

Must have prior HH experience, Oasis and HCHB or training before start required

Skills
Current Nursing License in the Practicing State
Valid Drivers License
Current CPR Certification
Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Education
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
