The Licensed Practical Nurse (LPN) responsibilities include-
- Maintain and document clients medical necessity.
- Admit and Discharge clients.
- Complete Admission documentation for clients enrollment.
- Manage clients Med-list upon admission through discharge.
- Work with Clinical team on clients treatment goals.
- Conduct education groups with proper documentation as needed.
- Monitor and document the effectiveness of treatment and medication.
- Maintain a courteous, and professional appearance at all times.
- Follow facility policy and procedures as outlined in the Employee Handbook.
- Follow a comprehensive plan of care for individual clients.
- Coordinate with Home Health facilities.
- Other duties may apply.
- To work from 6am-2pm and 4pm -10pm
