The Manager of Advance Practice Providers serves as a member of the PACE Interdisciplinary Team for planning, collaboration, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care within the designated service that they oversee. Incumbent assists the Director of Primary Care Operations in the APP administrative areas. This position is full time M-F 8am to 4pm with great work life balance and benefits!
Essential Responsibilities:
- Conducts comprehensive physical examinations as needed on a quarterly basis for the participant’s first year of enrollment and every six months thereafter.
- Evaluates participants as needed for acute and chronic illness management and performs follow-up visits from short term rehab or hospitalizations when indicated.
- Makes nursing home visits in accordance with the state nursing home requirements and as needed.
- Performs home visits as clinically indicated.
- Participates in the After Hours On-Call Coverage.
- Provides appropriate health care teaching to patients, their families and caregivers, as well as to other members of the Interdisciplinary Team.
- Completes appropriate, accurate, timely documentation, onsite and offsite.
- Ensures that duties are accomplished in a fiscally responsible manner adhering to established budgetary constraints.
- Ensures that all interactions with clients are conducted in a manner that adheres to the highest standards of care.
- Works in collaboration with the Quality Improvement team on designated programs and projects.
- Recruits, develops and motivates direct reports. Initiates and communicates a variety of personnel actions including employment, termination, performance reviews, salary reviews and disciplinary actions.
- Actively participates in Clinical management meetings and assigned committees.
- Provides coverage at other sites as needed based on clinical need
- Other duties as assigned by the Director of Primary Care Services or CMO
- Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
- Frequent local travel.
Job Specification:
- Master of Science Degree in Nursing (MSN) or Physician Assistant Studies with 5-10 years of NP or PA experience
- Previous supervisory experience preferred.
- Current Board Certification as a Nurse Practitioner or Physician Assistant in the Commonwealth of Massachusetts.
- Current licensure in Massachusetts
- Massachusetts and DEA Prescription Privileges.
- Credentialed for the Element Care affiliate Long Term Care Facilities.
- Current CPR Certification.
- Minimum of 5 years working with a geriatric population with PACE model required.
- Ability to convey information clearly and succinctly with all employees, so that relevant information is communicated appropriately and in a timely manner.
- Ability to lead independently and to be able to work well with others.
- Ability to work within a team concept and to be considerate of co-workers and colleagues.
- Ability to pay attention to detail, to be a self-starter, and to complete tasks on time.
- Ability to be able to quickly recognize situations/problems before they become acute, and to be able to implement effective solutions and understand consequences.
