\nPosition Summary:
\nThe Licensed Clinical Social Worker is to provide rehabilitative and supportive casework in the patient’s home geared toward restoring patients to their optimum level of social and health function.

Position Duties:
\n- \n
- Supervise the assistant social worker \n
- Complete social work treatment plans through activities such as referral to other community-based programs, education, counseling, networking and advocacy \n
- Conduct reviews to determine the effectiveness of the social work plans and revise as necessary \n
- Participate with patients and their representatives in planning for after discharge care and follow-up care as applicable \n
- Educate other agency staff about appropriately dealing with patient's psycho-social needs \n
- Participate in community education activities \n
- Other duties as assigned \n
Other Requirements:

- Must be able to speak and write in English \n
- Unrestricted driver’s license \n
- Current automobile insurance according to Company policy \n
Licensing/Certification/Education/Experience Requirements:
\n- \n
- Master's Degree or Doctoral Degree in Social Work from a school accredited by the Council on Social Work Education \n
- One (1) year experience of social work within a healthcare setting \n
- Supervisory experience preferred \n
- Home health experience preferred \n
- Licensed Clinical Social Worker (LCSW) \n
- Current state license in social work \n
- Current state driver’s license \n
- CPR license \n
- Automobile insurance in accordance with Company policy \n
